Abstract
The aim of this study was to determine the prevalence of Moraxella catarrhalis in lower respiratory tract infections and the rates of carrier state in adults and children and also beta-lactamase production and susceptibility to various anti-microbial agents. Three-hundred-seventy-five sputum samples were obtained from patients with suspected lower respiratory tract infection. Two-hundred-fifty-one (67%) fulfilled direct examination criteria and were cultured. Ninty-nine (39%) of them were positive for pathogenic microorganisms. The most common pathogen was Streptococcus pneumoniae (18%) followed by Staphylococcus aureus (15%), Pseudomonas aeruginosa (13%) and M. catarrhalis (11%). Twenty-nine of ninety-nine culture positive patients had Chronic Obstructive Lung Disease (COLD) and in this group M. catarrhalis was the predominant microorganism (9/29, 31%) (p>0.05). Nine out of 11 (82%) M. catarrhalis isolates were beta-lactamase positive and resistant to penicillin and ampicillin. Resistance patterns of these nine strains suggested the presence of BRO1 in eight and BRO2 in one isolate(s). To investigate the colonization rates, oropharyngeal secretion (saliva) were collected from 100 healthy people. The incidence of colonization and beta-lactamase production of M. catarrhalis were 24% and 38%, respectively. The colonization rates of the organism were as follows :14/29 (48%) in the 0-10 age group, 4/45 (9%) in 10-60 age group, 6/26 (23%) in the 60 and over age group and beta-lactamase production rates were 6/14 (43%), 1/4 (25%) and 2/6 (33%) in these groups, respectively. These strains were also resistant to penicillin and ampicillin and resistance patterns of these strains suggested the presence of BRO1 in eight and BRO2 in one isolate(s).

Bu çalışmada alt solunum yolların infeksiyonlarında Moraxella catarrhalis'in prevalansı ve erişkin ile çocuklarda taşıyıcılık oranı, beta-laktamaz üretimi ve farklı antibakteriyel ajanlara karşı duyarlığının saptanması amaçlanmıştır. Alt solunum yolu infeksiyonu kuşkusu ile gönderilen 375 balgam örneğinden 251 (%67)'i direkt bakı incelemesinde uygun bulunup kültüre alınmıştır. Bu kültürlerden 99 (%39)'u patojen mikroorganizmalar yönünden olumlu bulunmuştur. En sık patojenler sırası ile Streptococcus pneumoniae (%18), Staphylococcus aureus (%15), Pseudomonas aeruginosa (%13) ve M. catarrhalis (%11) olmuştur. Balgam kültürü olumlu bulunan 99 hastanın 29'u kronik obstrüktif akciğer hastalığı olan hastalar olup bu grupta M. catarrhalis en sık izole edilen mikroorganizma olarak bulunmuştur (9/29, %31) (p>0.05). Onbir M. catarrhalis izolatından 9 (%82)'u beta-laktamaz üretmiş ve penisilin ile ampisiline dirençli bulunmuştur. Bu dokuz izolatın direnç paternleri incelendiğinde sekiz izolatta BRO1 ve bir izolatta ise BRO2 tipi beta-laktamaz enzimlerinin bulunduğu düşünülmektedir. Kolonizasyon oranını araştırmak amacı ile 100 sağlıklı kişiden orofaringeal sekresiyon (tükürük) örneği alınmıştır. Bu grupta M. catarrhalis'in kolonizasyon ve beta-laktamaz üretim oranı sırası ile %24 ve %38 olarak saptanmıştır. Kolonizasyon oranı 0-10 yaş arasında 14/29 (%48), 10-60 yaş arasında 4/45 (%9) ve 60 yaşın üstünde 6/26 (%23) ve aynı yaş gruplarında beta-laktamaz üretim oranı, sırası ise, 6/14 (%43), 1/4 (%25) ve 2/6 (%33) olarak saptanmıştır. Bu suşlar ayrıca penisilin ve ampisiline dirençli olup direnç paternleri bu suşların sekizinde BRO1 ve birinde BRO2 bulunabileceğini göstermiştir.
