Value of polymerase chain reaction in the detection of Helicobacter pylori in gastric biopsy specimens

Abstract

A number of tests are available for the detection of Helicobacter pylori, the etiologic agent for a variety of gastric and duodenal diseases. Histological examination and bacterial culture of gastric biopsy specimens have diagnostic value. Recently, the molecular methods such as Polymerase Chain Reaction (PCR) were developed to detect H. pylori DNA in gastric biopsy specimens. In the present study; PCR, culture, and Gram stain were used for H. pylori detection in gastric biopsy specimens obtained from 60 patients, and advantages and disadvantages of these three tests were discussed. The detection rates of H. pylori using PCR, culture, and Gram stain were 73.3%, 70%, 80%, respectively. When the infection was defined by positive culture, the PCR assay had a sensitivity of 95.2% and a specificity of 77.8% whereas Gram stain had a sensitivity of 90.4% and a specificity of 44.4%.

Özet

Birçok gastrik ve duodenal hastalığa neden olan Helicobacter pylori'nin belirlenebilmesi için çeşitli testler vardır. Hastadan endoskopi yardımı ile alınan gastrik biyopsi örneğinden direkt mikroskopik inceleme ve kültür yapılması tanı koydurucudur. Son yıllarda H. pylori DNA'sının gastrik biyopsi örneklerinde gösterilebilmesi amacıyla Polimeraz Zincir Reaksiyonu (PCR) gibi moleküler teknikler de kullanılmaya başlanmıştır. Bu çalışmada, dispeptik yakınmaları olan 60 hastadan alınan gastrik biyopsi örneklerine PCR, kültür ve Gram boyama yöntemleri uygulanarak her üç testin avantajları ve dezavantajları gözden geçirildi. Sırasıyla PCR ile %73.3, kültür ile %70, Gram boyama ile %80 oranında H. pylori saptandı. Kültür pozitifliği geçerli test olarak alındığında PCR'ın duyarlılığı %95.2, özgüllüğü %77.S; Gram boyamanın duyarlılığı %90.4, özgüllüğü %44.4 olarak belirlendi.
