Comparison of Hepatitis B virus (HBV) serological markers and existence of HBV DNA

Abstract
Hepatitis B Virus (HBV) exists commonly all over the world and causes chronic hepatitis, cirrhosis and hepatocellular carcinoma. For this reason, early diagnosis and proper treatment of the infection are of almost importance. Existence of HBV DNA is proven by means of molecular diagnostic methods. The aim of this study was co-assessment of HBsAg and antiHBc total positivity and existence of HBV DNA. Totally 155 serum samples were screened for the presence of HBV DNA with Hybrid Capture method and for hepatitis markers with Microenzyme Immune Assay. HBV DNA was positive in 20 % of 110 HBsAg positive and in 4.4% of 45 HBsAg negative serum samples. HBV DNA was +/- in 15.9 % of 110 HBsAg positive and in 8.8 % of 45 HBsAg negative serum samples. HBV DNA was positive in 62.5 % of 24 serum samples which were HBeAg positive and antiHBe negative, 5.5 % of 72 which were HBeAg negative and antiHBe positive, 40.0 % of five which were both HBeAg and antiHBe positive, 11.1 % which were negative for both. The positivity rate and the median of HBV DNA of HBeAg positive and antiHBe negative group were significantly higher than the HBeAg negative and antiHBe positive group. Among 40 serum samples which were negative for the serological indicators of HBV exposure (HBsAg, antiHBs, antiHBc), HBV DNA was not positive while HBV DNA was +/- in 12.0 %. HBV DNA positivity in antiHBs positive, only antiHBe and antiHBc positive and isolated antiHBc positive groups were, respectively, 0.0 %, 25.0 % and 10.0 %. According to the results of this study; detection of HBV DNA in serum is directly correlated with HBeAg seropositivity and for the evaluation of HBV DNA results lower than 10 pg/ml, monitorization of cases and more sensitive tests are necessary.

Bu çalışmada; Hepatit B Virus (HBV)'a ilişkin olarak HBsAg, HBeAg, antiHBe ve antiHBc total pozitifliği ile viral replikasyonun göstergesi olan HBV DNA'nın korelasyonunun değerlendirilmesi amaçlanmıştır. Toplam 155 hasta serum örneğinde HBV DNA Hybrid Capture yöntemi ile, hepatit markerleri ise mikroELISA kitleri ile tam otomatize sistemlerde araştırılmıştır. HBV DNA testinde 10 pg/ml ve üzeri olumlu, 1-10 pg/ml arasındaki değerler ise +/- olarak değerlendirilmiştir. HBV DNA, HBsAg olumlu 110 örneğin 22'si (% 20) ile, HBsAg olumsuz 45 örneğin ikisinde (% 4.4) pozitif, HBsAg olumluların 14'ünde (% 12.7), olumsuzların ise yedisinde (% 15.6) +/- değerlerde saptanmıştır. HBeAg olumlu, antiHBe olumsuz 24 örneğinin 15'inde (% 62.5), HBeAg olumsuz, antiHBe olumlu 72 örneğin dördünde (% 5.5), HBeAg ve antiHBe birlikte olumlu yedi örneğin ikisinde (% 40.0), HBeAg ve antiHBe'nin olumsuz olduğu dokuz örneğin birinde (% 11.1) HBV DNA olumlu bulunmuştur. HBeAg olumlu ve antiHBe olumsuz gruptaki HBV DNA'nın olumluluk oranı, HBeAg olumsuz antiHBe olumlu gruptakilere göre anlamlı olarak yüksektir. HBsAg olumsuz örneklerden HBV DNA ile karşılaşmayı gösteren HBsAg, antiHBc yada antiHBs'den hiçbirinin bulunmadığı 25 örneğin hiçbirinde HBV DNA 10 pg/ml üzerinde saptanmazken, % 12'sinde +/- değerlerde bulunmuştur. AntiHBs olumlu, sadece antiHBe ve antiHBc olumlu ve salt antiHBc olumlu örneklerde HBV DNA'nın olumluluk oranları sırası ile % 0.0, % 25.0 ve % 10.0'dur. Çalışma sonuçlarına göre; serumda HBV DNA varlığının HBeAg olumluluğu ile ilişkili olduğu ve 10 pg/ml'den düşük miktarların değerlendirebilmesi için olguların izlenmesi ve daha duyarlı testlerin gerekliliği düşünülmüştür.
