Abstract

Nonconvulsive status epilepticus (NCSE) is state seen more frequently than expected, characterized by changes in either behaviour or consciousness accompanied by simultaneous seizure activity recorded on electro-encephalography (EEG) and good response to antiepileptic drug therapy. The diagnosis of NCSE is either delayed or not made because behavioural or consciousness changes are often thought to be due to the underlying disease or psychiatric disorders. A 44-year-old women who was on cefepime therapy because of sepsis, developed behavioural and consciousness changes acutely. The EEG showed continuous spike and wave discharges, and she was diagnosed as NCSE. After cutting off cefepime, she improved completely, and NCSE was thought to be due to cefepime.

Özet

Nonkolvülzif status epileptikus (NKSE) sanıldığından sık görülen, davranış ya da bilinç değişikliğine elektro-ensefalografi (EEG) nöbet aktivitesinin eşlik ettiği ve antiepileptik tedaviye yanıt veren bir tablodur. Davranış ya da bilinç değişikliği çoğunlukla altta yatan hastalığa bağlandığı veya psikiyatrik tablolar akla geldiği için tanısı gecikir ya da konamaz. Kırk dört yaşındaki kadın hastada sepsis nedeniyle sefepim tedavisi görmekteyken akut davranış ve bilinç değişikliği gelişti. Elektro - ensefalogramında devamlı diken-dalga aktivitesi saptandı ve NKSE tanısı konuldu. Sefepim tedavisi sonladırıldıktan sonra tamamen düzelen hastada NKSE'nin sefepime bağlı olduğu düşünüldü.
