Evaluation of examining multiple sputum specimens in the diagnosis of pulmonary tuberculosis

Abstract

Recently the number of sputum samples required for the diagnosis of pulmonary tuberculosis has been discussed in terms of cost and time. Despite suggestions that the number of sputum samples sent to the laboratory should be decreased, up to six sputum samples from a single patient are seen in routine application. The purpose of the present study was to determine the value of the number of sputum specimens in the diagnosis of pulmonary tuberculosis. Totally 6844 respiratory specimens sent to the mycobacteriology laboratory between January 1998 and May 2001 were evaluated retrospectively. All of the specimens were examined by acid-fast bacilli (AFB) smear and BACTEC 460 TB culture system. Seven hundred and eighty five (11%, 785/6844) specimens of 353 patients were positive for Mycobacterium tuberculosis complex. For 76% (270/353) of these patients the organism was detected in one or more sputum specimens. Of the patients with sputum specimens submitted to the laboratory, 16% (42/270) had only a single specimen sent for examination, 11% (29/270) had two specimens collected, and 74% (199/270) had at least three specimens sent to the laboratory. The AFB smear was found positive for 55% (23/42) of patients from whom only a single sputum specimen was sent whereas the AFB was detected at least once for 55% (16/29) of patients who had two specimens sent to the laboratory. Smear for AFB was positive for 69% (137/199) of the patients who had three or more sputum specimens collected. Mycobacterium tuberculosis was isolated in the first, second and third samples of the majority (98%, 195/199) of the patients giving three or more sputum samples. The results indicated that M. tuberculosis isolation could be carried out in the first, second and third sputum samples of the great majority (98%) of the patients and that the diagnostic value of four or more sputum specimens submitted to the laboratory was very low (2%).

Akciğer tüberkülozunun tanısında gerekli olan balgam örneklerinin sayısı, son yıllarda maliyet ve zaman açısından tartışılmaktadır. Laboratuvara gönderilen balgam örneklerinin sayısının azaltılması gerektiğini ileri süren araştırıcılar yanısıra, rutin uygulamada bazen altıya varan balgam örneği gönderen klinisyenler görülmektedir. Çalışmada, akciğer tüberkülozu tanısında balgam örneği sayısının değerini belirlemek amacı ile, Ocak 1998-Mayıs 2001 tarihleri arasında mikobakteriyoloji laboratuvarına gönderilen 6844 solunum sistemi örneği retrospektif olarak değerlendirilmiştir. Örneklerin tümü asido-rezistan bakteri (ARB) yayma ve BACTEC 460 TB kültür sistemi ile incelenmiş, 353 hastanın toplam 785 örneği (% 11, 785/6844) Mycobacterium tuberculosis kompleks yönünden pozitif bulunmuştur. Bu hastaların %76 (270/353)'sının bir veya daha fazla balgam örneğinde organizma saptanmıştır. Hastaların %16 (42/270)'sından sadece bir, %11 (29/270)'inden sadece iki, %74 (199/270)'ünden ise en az üç balgam örneği gönderilmiştir. Sadece bir balgam örneği gönderilenlerin %55 (23/42)'inde ARB yayma pozitifliği bulunurken, sadece iki ve üç veya daha fazla sayıda balgam örneği gönderilenlerde ise sırasıyla %55 (16/29), %69 (137/199) oranında en az bir defa ARB yayma pozitifliği saptanmıştır. Üç veya daha fazla sayıda balgam örneği gönderilen hastaların (%98,195/199) büyük çoğunluğunun birinci, ikinci ve üçüncü örneklerinden M. tuberculosis izole edilmiştir. Bu bulgular, hastaların büyük çoğunluğunda (%98) birinci, ikinci ve üçüncü balgam örneklerinden M. tuberculosis izolasyonunun gerçekleştirilebildiğini, dördüncü ve daha fazla gönderilen balgam örneklerinin ise tanı değerinin oldukça düşük (%2) olduğunu göstermektedir.
