An investigation on the prevalence of spondylitis and spondylodiscitis in brucellosis by using magnetic resonance imaging

Abstract
The purpose of this study was to determine the prevalence of spondylitis/spondylodiscitis in brucellosis and also to determine the place of magnetic resonance imaging (MRI) method in diagnosing these complications Brucellar spondylitis and spondylodiscitis in 50 patients hospitalized in the Clinic of Infectious Diseases, Atatürk State Hospital, Izmir, were investigated by using MRI. Spondylitis/spondylodiscitis was found in 14 patients by using MRI although only 4 of them had positive imaging findings on plane radiograms. MRI was found to be a more sensitive method to detect the presence of spondylitis/spondylodiscitis than plane radiograms (p=0.002). Since brucellar spondylitis is a long lasting clinical entity needing long term therapy and causes considerable suffering and work loss, it is concluded that all brucellosis patients suffering from neck or lumbal pain either with or without positive imaging findings on plane radiograms should also be examined with MRI.
Özet
Bu çalışmanın amacı, brusellozda spondilit/spondilodiskit prevalansını ve bu komplikasyonların tanısında magnetik rezonans görüntüleme (MRG) yönteminin yerini saptamak idi. İzmir Atatürk Devlet Hastanesi Enfeksiyon Hastalıkları Kliniği'nde bruselloz tanılı 50 hastada MRG ile spondilit/spondilodiskit prevalansı araştırıldı. Hastaların 14 (%28)'ünde MRG ile spondilit/spondilodiskit saptanırken sadece dördünde (%8) direkt grafide spondilit/spondilodiskitten kuşkulandıracak bulgular saptandı. Spondilit/ spondilodiskit varlığını araştırmada MRG yönteminin direkt grafiye göre anlamlı olarak daha duyarlı olduğu bulundu (p=0.002). Sonuç olarak, Brucella spondilitinin sinsi seyri, uzun süreli iş gücü kaybına neden olması ve tedavisinin uzun süre devam etmesi gerektiğinden bel/boyun ağrısı olan tüm brusellozlu hastalara, direkt grafi bulgusu olsun veya olmasın, MRG yönteminin uygulanması gerektiği kanısına varıldı.
