Vancomycin tolerance in enterococci

Abstract

Tolerance can be defined as the ability of bacteria to grow in the presence of high concentrations of antimicrobials, where the killing action of the drug is avoided but the MIC (Minimum Inhibitory Concentration) remains the same. The purpose of this study was to determine the vancomycin tolerance in enterocöcci. Enterococcal strains isolated from different clinical specimens were identified and MIC and MBC (Minimal Bactericidal Concentration) values of vancomycin were detected in 44 and 56 of the enterococcal strains'. Of the strains 56 and 44 of the were identified as E. faecium and E. faecalis, respectively. Thirthy-one E. faecium and 48 E. faecalis strains were found to be susceptible to vancomycin and these susceptible strains were included in the study. To determine MICs and MBCs, strains were inoculated from an overnight agar culture to MHB (Müller-Hinton Broth) and shaken for 4-6 h at 37° C to obtain a logarithmic phase culture. The inoculum was controlled by performing a colony count for each test. The MBC/ MIC ratio was used to detect tolerance to vancomycin. Vancomycin tolerance was indicated by MBC/MIG ratio equal and greater than 32. Tolerance to vancomycin was detected in 48 E. faecalis and 31 E. faecium strains. Standard E. faecalis 21913 strain also showed tolerance to vancomycin. The MICs of susceptible strains ranged from 512 |xg/ml. A 100% tolerance to vancomycin was found in susceptible enterococcal strains. One of the hypothese for tolerance is that tolerant cells fail to mobilize or create the autolysins needed for enlargement and division. The results of this study suggests that tolerance maycompromise vancomycin therapy of serious enterococcal infections. To add an aminogiycoside to the giycopeptide therapy unless MBCs are unavailable can be useful in effective treatment of enterococcal infections.

Tolerans bakterilerin yüksek antibiyotik konsantrasyonlarında dahi üreyebilmesi olarak adlandırılabilir. Böylece bakteriler antibiyotiklerin öldürücü etkilerinden etkilenmeyecek, ancak bununla birlikte MİK (Minimal inhibitor Konsantrasyon) değerleri aynı olarak kalacaktır. Çeşitli klinik örneklerden izole edilen 100 enterokok kökeni tiplendirilmiş ve 44'ü Enterococcus faecium ve 56'sı E. faecalis olarak bulunan enterokok kökenlerinde vankomisinin MİK ve MBK (Minimal Bakterisidal Konsantrasyon) değerlerinin saptanması amaçlanmıştır. Vankomisine karşı duyarlı bulunan 31 E. faecium ve 48 E. faecalis kökeni vankomisine karşı duyarlı olarak bulunmuş ve çalışmaya alınmıştır. MİK ve MBK değerlerini saptamak için, bir gecelik üremeden sonra suşlar MHB (Müller-Hinton Broth)'a aktarılmış ve 37° C'de 4-6 saat, logaritmik üreme fazı elde edilinceye kadar çalkalanmıştır. Her testte kullanılan inokulum, koloni sayımı ile kontrol edilmiştir. Vankomisin toleransını saptamak için MBK/MİC oranı kullanılmıştır. MBK/MİK oranı 32 veya daha yüksek olan kökenler vankomisine karşı "toleran" olarak kabul edilmişlerdir. Tolerans çalışması yapılan 31 E. faecium ve 48 E. faecalis kökeninde, vankomisine karşı antibiyotik toleransı saptanmıştır. Standart E. faecalis 21913 kökeni de vankomisine karşı tolerans göstermiştir. Duyarlı kökenlerin MİK değerleri 512 |xg/ml olarak bulunmuştur. Sonuç olarak, duyarlı enterokok kökenlerinin %100'ünde vankomisine karşı tolerans saptanmıştır. Bir hipoteze göre, toleran bakteriler büyümeleri ve bölünmeleri için gerekli olan otolizinleri oluşturamamakta veya harekete geçirememektedir. Elde edilen sonuçlarda, ciddi enterokok infeksiyonlarında, vankomisin toleransının etkili bir tedaviyi güçleştirdiğini düşündürmektedir. MBK'nın elde edilemediği durumlarda etkili bir tedavi için glikopeptit tedavisine bir aminoglikozit eklemek yararlı olabilir.
