Diplopia as the first sign of brucellosis and review of cranial nerve involvement

Abstract
Brucellosis in a common public health problem in Turkey especially in rural areas. Neurobrucellosis is a rare complication of brucellosis. A 33-year-old male shepherd was hospitalized with headache, diplopia, paresia of both upper and lower extremities, and EMG findings of mixed polyneuropathy with sensorial component. Based on physical examination and laboratory findings, the patient was diagnosed as neurobrucellosis. Ceftriaxon 4 g IV and rifampicin 600 mg PO per day were given for 28 days. The patient was discharged on rifampicin 600 mg PO per day and doxycyline 100 mg PO bid for the next four months. In his follow-up visits, he had no complaint.
Özet
Bruselloz Türkiye'de özellikle kırsal kesimde yaşayanlar arasında önemli bir halk sağlığı problemidir. Nörobruselloz brusellozun nadir bir komplikasyonudur. Otuz üç yaşında, mesleği çoban olan, erkek hasta başağrısı, çift görme, her iki alt ve üst ekstremitede uyuşma ve EMG'de sensoriyal komponenti olan mikst tipte polinöropati bulguları ile kliniğe yatırıldı. Fizik muayene ve laboratuvar bulguları ile nörobruselloz tanısı konuldu. Seftriakson 4 g İV/gün ve rifampisin 600 mg/gün oral yoldan 28 gün boyunca verildi. Hasta rifampisin 600 mg/gün ve doksisiklin 100 mg günde iki kez oral yoldan alacak şekilde dört ay boyunca kullanmak üzere taburcu edildi. Hastanın kontrollerinde herhangi bir yakınması yoktu.
