The relation between arthritis and parvovirus B19 infection

Abstract

It was asserted that human parvovirus B19 which is responsible for the fifth disease, aplastic anemia and arthritis can play a role in the autoimmune diseases like rheumatoid arthritis (RA) and systemic lupus erythematosus (SLE). In order to evaluate the role of parvovirus B19 in the aetiopathogenesis of these diseases, synovial fluids and blood specimens were collected from 20 patients with early synovitis and 31 patients with RA at one month intervals. Blood specimens were collected also from 25 patients with SLE, 25 patients with osteoarthritis as the unhealthy control group and 50 healthy people as the healthy control group. ELISA was used for the detection of parvovirus B19 IgM and parvovirus B19 IgG in serum specimens and PCR for the detection of parvovirus B19 DNA in synovial fluid samples. Parvovirus B19 IgM, parvovirus B19 IgG and parvovirus B19 DNA were found in 3 patients of the early synovitis group, and later 2 of the 3 patients were diagnosed as RA and olie of the 3 patients was diagnosed as SLE. Parvovirus B19 DNA was detected in the synovial fluids of 8 patients in the RA group and, parvovirus B19 IgM and parvovirus B19 IgG. were found in the 4 patients who were detected as parvovirus B19 DNA (+) and parvovirus B19 IgG were detected as positive in the other 4 patients. Parvovirus B19 IgM positivity was not detected in both control groups. The results seem to be in accordance with the theory that parvovirus B19 may have a role in the etiopathogenesis of RA in the case level. New extensive serial studies are needed on the etiopathogenetic relation between parvovirus B19 and rheumatic diseases such as RA and other chronic inflammatory and autoimmune diseases.

Çocuklarda ve yetişkinlerde beşinci hastalık, artrit ve aplastik anemi gibi hastalıklardan sorumlu olduğu bilinen parvovirus B19'un romatoit artrit, sistemik lupus eritematozus (SLE) gibi oto-immün karakterli hastalıklarda rol oynayabildiği ileri sürülmektedir. Bu hastalıklarda parvovirus B19'un etyopatogenezdeki rolünü değerlendirmek amacıyla 20 erken sinovitli, 31 romatoit artritli hastadan eklem sıvısı ve birer ay ara ile kan örnekleri toplanmıştır. Çalışmada ayrıca hastalıklı kontrol grubu olarak 25 SLE'Iİ, 25 osteo-artritli hastadan ve sağlıklı kontrol grubu olarak 50 sağlıklı kişiden kan örnekleri toplanmıştır. Serum örneklerinde ELISA ile parvovirus B19 IgM ve parvovirus B19 IgG saptaması, eklem sıvısı örneklerinde ise polimeraz zincir reaksiyonu yöntemi ile parvovirus B19 DNA araştırması yapılmıştır. Erken sinovit grubunda üç hastada hem parvovirus B19 IgM hem parvovirus B19 IgG hem de parvovirus B19 DNA saptanmış olup, sonradan bu hastalardan ikisine romatoit artrit, birine de SLE tanısı konulmuştur. Romatoit artrit grubunda sekiz hastanın eklem sıvısında parvovirus B19 DNA saptanırken, parvovirus B19 DNA (+) saptanan hastalardan dördünde parvovirus B19 IgM ve parvovirus B19 IgG, diğer dördünde sadece parvovirus B19 IgG (+) olarak belirlenmiştir. Ayrıca hastalıklı ve sağlıklı kontrol gruplarında parvovirus B19 IgM yanıtı saptanmamıştır. Çalışma sonuçları; parvovirus B19'un romatoit artrit etyopatogenezinde rolü olabileceğini öne süren görüşlerle olgu düzeyinde paralellik göstermiş olsa da, gerek romatoit artrit gerekse diğer kronik inflamatuvar ve oto-immünite ile karakterize romatizmal hastalıklarla parvovirus B19 arasındaki etyopatogenez ilişkisine yönelik geniş serili yeni çalışmalara gereksinim olduğu görülmektedir.
