Investigation of stethoscopes in terms of bacterial contamination

Abstract

The probability of contamination or spread of infectious agents, particularly bacteria, in hospitals may occur via the use of stethoscopes that are in direct contact with patients. The aim of the present study to investigate the role of the diaphragms of stethoscopes in terms of bacterial contamination. For this purpose, culture specimens from 107 stethoscope diaphragms (of 69 physicians and 38 nurses) were obtained by pressing them directly onto blood agar and EMB agar plates. It was found that about two thirds of the stethoscopes (69.2%) were contaminated by one or more bacteria. The most frequent isolates obtained were coagulase-negative staphylococci (65.4%) followed by diphtheroids (5.6%), Bacillus subtilis (%2.8%) and Acinetobacter spp. (0.9%). There was not any significant difference between the physicians' and nurses' stethoscopes with respect to bacterial contamination (p>0.05). It was concluded that stethoscopes could be vectors for nosocomial infections and bacterial contamination may spread via use of uncleaned stethoscopes.

Özet

Hasta muayenesinde yaygın olarak deriye direkt temas ettirilerek kullanılan stetoskopların, hastanede özellikle bakteri infeksiyonu etkenlerinin yayılmasına aracılık edebilir. Bu çalışmada, stetoskop diyaframlarının bakteri kontaminasyonu yönünden incelenmesi yapılarak bakterilerin taşınmasındaki rolünün araştırılması amaçlanmıştır. Rutin kullanım sırasında herhangi bir temizlik işlemi yapılmadan 69 doktor ve 38 hemşireye ait stetoskobun diyaframı % 5 koyun kanlı ağar ve eozin metilen blue ağar plakları üzerine temas ettirilerek kültür örnekleri alınmış, incelenen stetoskopların % 69.2'si bir ya da birden fazla bakteri türüyle kontamine olduğu saptanmıştır. Stetoskoplardan en sık olarak koagülaz-olumsuz stafilokoklar (%65.4); daha az olarak difteroit çomaklar (%5.6), Bacillus subtilis (%2.8) ve Acinetobacter türleri (%0.9) izole edilmiştir. Doktor ve hemşire stetoskopları arasında bakteri kontaminasyonu yönünden anlamlı bir fark saptanmamıştır (p>0.05). Çalışmada, stetoskopların bakterilerle kolonize olduğu saptanmıştır. Bu bakterilerin stetoskoplar aracılığı ile hastadan hastaya taşınabileceğini ve buna bağlı olarak hastane infeksiyonlarının oluşmasına katkıda bulunabileceği düşünülmüştür.
