Brucellar septic arthritis in a diabetic patient

Abstract
A 72-year-old female patient with brucellar septic monoarthritis is presented with radiological findings and treatment results. The patient received streptomycin (1g/day) for 21 days, ciprofloxacin (1000 mg/day), and doxycycline (200 mg/day) for 45 days. At the end of treatment, the patient's complaints completely resolved, and magnetic resonance imaging findings completely disappeared. To the best of our knowledge, this is the first report of septic arthritis caused by Brucella melitensis in a diabetic patient. The case showed that 45-day treatment with three antibiotic combination provided complete clinical and radiologic recovery in brucellar arthritis accompanied by Diabetes mellitus.
Özet
Bu çalışmada, Brucella septik mono-artriti olan diyabetik bir kadın hasta, radyolojik bulgular ve tedavi yönü ile sunuldu. Yetmiş iki yaşında diyabetik kadın hastaya Brucella artriti tanısıyla streptomisin 1g/gün, doksisiklin 200 mg/gün ve siprofloksasin 1g/gün başlandı. Streptomisin tedavisi 21. günde, diğer antibiyotikler 45. günde sonlandırıldı. Tedavi sonunda semptom ve bulguları ile birlikte 45. günde yapılan kontrol magnetik rezonans görüntüleme bulgularının tamamen kaybolduğu gözlendi. Eldeki bilgilere göre bu olgu, diyabetik hastada Brucella melitensis'in neden olduğu ilk septik artrit olgusudur. Bu olguda, 45 gün süreli, üçlü antibiyotik tedavisinin klinik ve radyolojik iyileşme yönünden yeterli olabileceği görülmüştür.
