Detection of antibodies against Coxiella burnetii in risk groups for Q fever: A study with ELISA and IFA tests

Abstract
Q fever is a zoonotic infection and is more frequently seen among risk groups in contact with animals. The purpose of this study was to determine the prevalence of Q fever in risk groups in Aydın Region, Turkey. For this purpose antibodies against Coxiella burnetii in risk groups were detected using enzyme linked immunosorbent assay (ELISA) and indirect fluorescent antibody test (IFA). A total of 92 people, including veterinarians, cattle-dealers, and butchers were included in the study. The study group filled survey forms on the risk factors and symptoms of Coxiella infection. Collected serum samples were investigated for C. burnetii Phase II IgG and IgM antibodies with ELISA in Microbiology and Clinical Microbiology Laboratories, Medical Faculty, Adnan Menderes University. All positive and equivocal results with ELISA were tested again with IFA for C. burnetii Phase I and Phase II IgM and IgG antibodies. With ELISA C. burnetii IgM and IgG were positive or equivocal in 20 (21.7%) and 41 (44.5%) samples, respectively. Among 20 IgM positive samples with ELISA, 7 (7.6%) were also positive for C. burnetii Phase II IgM with IFA. Of 41 IgG positive cases with ELISA, 39 (42.3%) were also positive for C.burnetii Phase II IgG with IFA but only one person was positive for C. burnetii Phase I IgG with IFA. In conclusion, high frequency of C. burnetii seropositivity was observed among risk groups. Coxiella burnetii infection should be considered for patients in contact with animals.

Q humması zoonotik bir infeksiyon olup, hayvanlarla temas içinde olan risk gruplarında daha sık görülmektedir. Bu çalışmada risk gruplarında Coxiella burnetii'ye karşı oluşan antikorlarının, böylelikle Q Humması prevalansının saptanması amaçlanmıştır. Veteriner hekimler, celepler ve kasaplar olmak üzere üç grupta toplam 92 kişi çalışmaya alınmış ve C. burnetii infeksiyonu ile ilgili risk faktörleri ve semptomları sorgulayan anket formu uygulanmıştır. Toplanan serum örneklerinde Adnan Menderes Üniversitesi Tıp Fakültesi Mikrobiyoloji ve Klinik Mikrobiyoloji Anabilim Dalı Laboratuvarı'nda enzyme linked immuno sorbent assay (ELISA) ile Faz II IgG ve IgM antikorları araştırılmıştır. ELISA ile pozitif veya kuşkulu saptanan tüm olgularda indirekt floresan antikor (IFA) yöntemi ile Faz I ve Faz II IgM ve IgG antikorları araştırılmıştır. ELISA ile 12 serumda (%13) C. burnetii IgM antikorları pozitif, sekiz (%8.7) serumda kuşkulu bulunmuştur. Toplam 20 kişide yapılan IFA Faz II IgM testi yedi (%7.6) olguda pozitif saptanmıştır. C. burnetii IgG antikorları ELISA ile olguların 32'sinde (%34.8) pozitif, dokuzunda (%9.7) şüpheli bulunmuştur. Toplam 41 kişide uygulanan IFA testiyle, C. burnetii Faz II IgG antikorları 39 (%42.3) kişide pozitif bulunmuştur. Bir kişide (%2.4) IFA ile C. burnetii Faz I ve Faz II IgG birlikte pozitif saptanmıştır. Sonuç olarak, Aydın bölgesinde risk gruplarında C. burnetii seropozitifliği yüksek oranda saptanmıştır. C. burnetii'nin neden olduğu klinik tablolarda özellikle hayvan teması öyküsü varsa, olası etkenler arasında araştırılması gerektiği düşünülmektedir.
