The Effect of Vaginal Bleeding on Parameters Used in the First Trimester Fetal Aneuploidy Screening Test

Abstract

Objective: The aim of the present study was to investigate the effect of vaginal bleeding on biochemical parameters of the first trimester fetal aneuploidy screening test.Material and Methods: The study population consisted of 11-14week- singleton pregnancies who were hospitalized for vaginal bleeding with the diagnosis of abortus imminens and had first trimester aneuploidy screening test in a perinatology clinic between 2009 and 2013. A control group of patients with similar gestational ages (11-14 wks) and singleton pregnancies without any vaginal bleeding, and systematic disease who underwent first trimester screening tests were also included in the analysis. The study and control groups were compared in terms of maternal age, maternal weight, gestational age, pregnancy history and first trimester screening test parameters (i.e. nuchal translucency (NT), crown-rump-length (CRL), free ?-hCG, PAPP-A). P values less than 0.05 were considered to be statistically significant. Results: A total of 139 pregnant women with active vaginal bleeding when the first trimester screening test was performed was included in the study group, and 453 healthy pregnant women with similar gestational ages were included in the control group. There was no statistically significant difference between the groups in terms of maternal age, weight, gravidity, parity, abortion, live birth, gestational age, NT, and CRL measurements (p>0.05). When the groups were compared in terms of PAPP-A and free ?-hCG MoM levels, a significant difference was found between the two groups (p=1/250), there was no significant difference between the groups (p>0.05). Conclusion: Active vaginal bleeding may interfere with the biochemical parameters of the first trimester aneuploidy screening test, and should be considered when evaluating the test results. In the light of the further large-scale studies to be performed, it will be appropriate to arrive at a conclusion discouraging the application of screening tests at the time of abortus imminens.

ET Amaç: Çalışmanın amacı ilk trimesterde mevcut olan aktif vajinal kanamanın ilk trimester fetal anöploidi tarama testi parametrelerine olan etkisinin incelenmesidir.Gereç ve Yöntem: Çalışmamıza, 2009 ve 2013 tarihleri arasında 11.-14. hafta arası, perinatoloji kliniğinde düşük tehdidi tanısıyla yatan, kanaması olan ve bu aralıkta ikili tarama testi alınan gebeler çalışma grubu olarak dâhil edildi. Kontrol grubu olarak tekil gebeliği olan ek hastalığı bulunmayan, ikili tarama testi yaptıran ve ilk trimesterde kanama öyküsü bulunmayan gebelerden oluşturulmuştur. Gruplar maternal yaş, maternal kilo, gestasyonel hafta, gebelik öyküsü, ikili test parametreleri olan Nukhal translüsensi (NT), Baş-popomesafesi (CRL), serbest beta human koryonik gonadotropin (serbest ?-hCG) ve Gebelikle İlişkili Plazma Protein-A (PAPP-A) Multiples of Median (MoM) seviyeleri açısından karşılaştırıldı. P değeri 0,05). Akut kanaması olan gebeler kontrol grubuyla karşılaştırıldığında PAPP-A ve serbest ?-hCG MoM seviyeleri anlamlı olarak çalışma grubunda yüksek olarak saptandı (p0,05).Sonuç: İlk trimester tarama testi yapılması planlanan gebelerde aktif kanamanın bulunması test biyokimyasal parametrelerini etkileyebileceğinden, taramanın kanamanın olmadığı bir zamanda yapılması gerektiği sonucuna ulaşılmıştır. Yapılacak daha büyük çalışmalar ışığında, düşük tehdidi sırasında ikili tarama testinin yapılmaması görüşüne varılması uygun bir seçenek olacaktır.
