Anjiografi ile koroner arter hastalığı tanısı konan hastalarda serum lipid profili

Abstract
Serum triglyceride (TG), total cholesterol (TC), phospholipid (PL), Apo A-l, Apo B, Lipoprote¬in (a) (Lp(a)), high-density lipoprotein cholesterol and its subunits (HDL-C, HDL2- C, HDL3-C), low-density lipoprotein cholesterol (LDL-C) and free cholesterol (PC) levels of 108 subjects with and without angiographically defined coronary artery diseases (CAD) were investigated. Patients were classified into three groups as one, two and three vessel diseases according to the number of vessels with over 50% stenosis. The mean levels ofLp(a), TC, LDL-C, and Apo B and LDL-C/HDL-C ratios of total patients group were significantly higher whereas Apo A-I and HDL-C values and HDL2-C/HDL-C, HDL-C/TC and HDL2-C/HDL3-C ratios were lower than those of controls. Lp(a) and TC levels and LDL-C/HDL-C ratio were significantly higher whereas HDL2-C level and HDL2-C/HDL3-C and HDL2-C/HDL-C ratios were significantly lower in all groups of patients than tho¬se of controls. Apo A-l, Apo B, LDL-C, HDL-C levels and HDL-C/TC ratio were different only in pa¬tients with multiple vessel disease. There was no significant difference in other parameters. It was concluded that the importance of risk factors, Lp (a) and HDL2-C levels HDL2-C/HDL-C and HDL2-C/HDL3-C ratios were more increased with the number of vessels affected, according to the other lipid parameters . Therefore, the combination of these factors besides conventional lipid pa¬rameters would suggest the severity and prevalence of multiple vessel disease.

Anjiografi ile koroner arter hastalığı (KAH) tamsı konan veya sağlıklı olduğu tesbit edilen 108 vakada, serum trigliserid(TG), total kolesterol (TK), fosfolipid (FL), apolipoprotein A-l(Apo A-l), apolipoprotein B (Apo-B), lipoprotein (a) (Lp (a)), yüksek dansiteli lipoprotein kolesterol ve alt fraksiyonları (HDL-K, HDL2-K, HDL3-K), düşük dansiteli lipoprotein kolesterol (LDL-K) ve serbest kolesterol (SK) seviyeleri araştırıldı, %50'nin üzerinde damar tıkanıklığı olan hastalar tıkanan ■ damar sayısına göre bir damar, iki damar ve üç damar olmak üzere üç gruba ayrıldı. Toplam, hasta grubunda kontrole göre Lp(a), TK, LDL-K ve Apo B seviyeleri ve. LDL-K/ HDL-K oranı anlamlı derecede yüksek bulunurken, Apo A-l ve HDL-K seviyeleri ile HDL2-K/HDL-K, HDL-K/TK ve HDL2-K/HDL3-K oranları düşük bulundu. Lp(a) ve TK seviyeleri ile LDL-K/HDL-K oranı her üç hasta grubunda kontrole göre önemli derecede yüksek bulunurken, HDL2-K seviyesi ile HDL2-K/HDL3-K ve HDL2-K/HDL-K oranlan düşük bulundu. Apo A-l, Apo B, LDL-K, HDL-K seviyeleri ve HDL-K/TK oranı sadece iki ve üç hasta gruplarında farklı idi. Diğer parametreler arasında an¬lamlı bir fark yoktu. Bulgularımız, KAH için risk faktörlerinden olan Lp(a), HDL2-K seviyeleri ile HDL2-K/HDL-K ve HDL2-K/HDL3-K oranları arasındaki farkın öneminin etkilenen damar sayı¬sı ile birlikte diğer lipid parametrelerine göre daha fazla arttığını, dolayısı ile bu risk faktörleri¬nin rutin lipid parametrelerine ilavesinin hastalığın ciddiyetini ve tutulan damar sayısının pre-valansını belirlemede daha yararlı olabileceğimi göstermiştir.
