The Role of Midwife and Nurse in the Care of Pregnant Women with Addison's Disease

Abstract
Addison’s disease is a chronic disease that occurs when the adrenal glands are damaged. Adrenal diseases are critical in the sense that they may cause serious maternal morbidity and mortality during pregnancy. The diagnosis of Addison's disease is difficult in pregnancy because its symptoms are similar to other physiological signs of pregnancy, which may delay the diagnosis. Furthermore, the detection of the disease by diagnosis is crucial to avoid fetal and maternal health risks. The fetuses of pregnant women with Addison’s disease may face the risk of adrenal insufficiency, preterm birth, abortion, low birth weight, and intrauterine growth retardation. An Addison’s disease crisis may occur due to stressful situations, progression of Addison’s disease, infectious diseases, excessive vomiting, and acute hypotension. Therefore, increasing treatment as well as midwifery and nursing care that will prevent these conditions should not be ignored in order to prevent the development of Addison’s disease. As a result, although Addison’s disease in pregnancy is not widespread, it may lead to critical problems due to the high risk. Therefore, this review was conducted in order to guide midwives and nurses when giving care to pregnant women.

Addison hastalığı sürrenal bezlerin destrüksiyonu ile giden kronik birhastalıktır. Adrenal hastalıklar, gebelikte maternal ve fetal ciddi morbiditeve mortalite nedeni olabilmeleri nedeniyle önemlidir. Addisonhastalığına tanı koymak gebelikte zordur çünkü bu hastalığa ilişkinsemptomlar gebeliğin fizyolojik belirtileri ile büyük oranda benzemektedir.Ayırıcı tanı ile hastalığın saptanması fetal ve maternal riskler açısındanönemlidir. Addison hastalığı olan gebelerin fetüslerinde adrenalyetmezlik, erken doğum, abortus, düşük doğum ağırlığı ve intauteringelişme geriliği riski mevcuttur. Yaşanan stresli durumlar, Addisonhastalığının ilerlemesi, infeksiyon, aşırı kusma ve akut hipotansiyonnedeniyle Addison krizi gelişebilmektedir. Bu nedenle yaşanan bu durumlardaAddison krizi gelişimini engellemek için tedavi rejimindekideğişikliklerin yanı sıra bu durumların görülmesini engelleyici ebelikve hemşirelik bakımı göz ardı edilmemelidir. Sonuç olarak, gebelikteAddison hastalığı yaygın görülmemesine rağmen, taşıdığı risk açısındanbüyük sorunlara yol açabileceği için bu derleme gebelere verilecekolan bakımda ebe ve hemşirelere rehber olabilmek amacıyla yazılmıştır.
