A case of rickettsial disease

Abstract

Although the rickettsial diseases are not seen very frequently in our country, they can't be recognised and can be missed even when they are present because some have abortive course. Here we present a patient who came with high fever, headache and maculopapular rashes on his trunk at the beginning. The rashes spread to his extremities, palms and soles later and some acquired petechial character. Regarding to this clinical picture and the history, rickettsiosis was thought and the Weil-Felix reaction and the skin biopsy was used to support our diagnosis. As a result, the patient was diganosed as Rickettsia conorii and treated efficiently.

Özet

Risketsial hastalıklar ülkemizde çok sık görülme¬mekle beraber, görüldüğünde de bir kısmı abortif seyret¬tiği için tanınamayan ve atlanabilen bir hastalıktır. Biz burada, yüksek ateş, başağrısı ve başlangıçta gövdede makülopapüler döküntüler ile gelen bir hastayı sunuyo¬ruz. Bu hastanın döküntüleri daha sonra ekstremitelere, avuç içi ve ayak tabanlarına yayılmış ve bazıları peteşi-el bir karakter kazanmıştır. Hastanın bu klinik tablosu ve öyküsü çerçevesinde riketsiyoz düşünülmüş ve tanı¬mızı desteklemek için Weil-Felix testi ve cilt biopsisinen yararlanılmıştır. Sonuçta da hasta R. conorii tanısı ola¬rak etkili bir şekilde tedavi edilmiştir.
