56 enterik ateş olgusunda kültür ve Gruber-Widal aglutinasyonu sonuçları

Abstract

The cultures and the Widal test results of 56 Enteric fever cases who were treated in Haydarpaşa Numune Hospital, Department of Infectious Disease and Clinical Microbiology were evaluated. Salmonella typhi was isolated from the blood of 18 (%36) and stool of 6 (%12) of 50 typhoid fever cases. Widal reaction were found to be positive in 45 cases. In two of typoid fever cases, Widal reaction was negative, despite the isolation of S. typhi from blood. Salmonella paratyphi B was isolated from the blood of 2 (%33) and the stool of 2 (%33) of 6 paratyphoid fever cases. Despite the isolation of S. paratyphi B from the blood, Widal reaction was found to be negative in one case. It is concluded that, Widal reaction can be negative in some of enteric fever cases and using Widal reaction alone in the diagnosis may cause wrong diagnose for the patients who really have enteric fever.

Özet

1991-1994 yıllan arasında, kliniğimizde enterik ateş olarak izlenen 56 olgunun kültür ve Gruber-Widal aglütinasyon testi sonuçlan değerlendirilmiştir. Tifo tanısı konulan 50 olgunun 18 (%36)'inde hemokültürde, 6 (%12)'sında koprokültürde Salmonella typhi üremiş; 45 (%90) olguda ise Gruber-Widal aglutinasyonu pozitif saptanmıştır. 2 (%11) olguda hemokültürde S. typhi üremesine karşılık, Gruber-Widal aglutinasyonu negatif devam etmiştir. Paratifo tanısı konulan 6 olgunun ise 2 (%33)'sinde hemokültürde, 2 (%33)sinde koprokültürde Salmonella paratyphi B üremiş; 5'inde Gruber-Widal aglutinasyonu pozitif saptanmıştır, l olguda ise kültürde Salmonella paratyphi B üremesine karşın, Gruber-Widal aglutinasyonu negatif saptanmıştır. Bazı enterik ateş olgularında Gruber-Widal aglutinasyonunun negatif olabileceği ve tanıda yalnızca bu test sonucuna göre hareket etmenin, gerçekte salmonelloz olan bazı olguların gözden kaçmasına neden olabileceği sonucuna varılmıştır.
