Erythropoetin levels in type II diabetes mellitus non-uremic patients with autonom neuropathy

Abstract
Autonom Neuropathy (AN) is a frequent complication of DM. Since erythropoetin production is partly regulated by autonomic nervous system, erythropoetin level may decrease in an early period in diabetic patients with AN. This can also explain the etiologic factor of anemia in diabetic patients. Cardiovascular tests were applied to 71 type II DM patients who doesn't have uremia or any pathological process leading to anemia and groups having and not having autonomic neuropathy were determined. The group without AN was accepted as control group. Then the AN group was subdivided into early, clinical, progressive and atypical subgroups. The severity of AN in early, clinical and progressive subgroups was increasing sequentially. The atypical subgroup was not fitting the criteria that used to identify AN and was formed from another combinations of abnormal tests. The Hb, Hct, red blood cell and erythropoetin levels of these subgroups were compared. According to these findings, as the severity of AN was increasing, Hb, Hct and red blood cell levels were decreasing. It was suggested that in patients with AN as the severity of autonomic neuropathy increases, the decreasing levels of erythropoetin production may be related to renal denervation.

Otonom nöropati (ON), diabetes mellitusun (DM) sık rastlanan bir komplikasyondur. ON'li diabetik hastalarda, eritropotein üretiminin kısmen otonom sinir sistemince düzenlenmesinden dolayı, eritropoetin üretimi erken dönemde bozulabilir. Bu da diabetli hastalarda görülen anemi nedenlerinden birini açıklayabilir. Bu noktadan hareketle, hastanemize başvuran nonüremik ve diabet dışında anemiye yol açabilecek hastalığı olmayan tip II DM'li 71 hastaya kardiyovasküler testler uygulanarak ON'si olan ve olmayan gruplar belirlendi. ON'siz grup kontrol grubu olarak kabul edildi. ON'li grup da erken, klinik, ilerlemiş ve atipik olmak üzere ayrıldı. Erken, klinik ve ilerlemiş grupta ON şiddeti sırasıyla giderek artıyordu. Atipik grup ise ON'i belirlemede kullanılan kriterlere uymayan ve anormal testlerin diğer kombinasyonlarından oluşan gruptu. Bu gruplar arasında Hb, Hct, eritrosit ve eritropoetin düzeyleri karşılaştırılmıştır. Bu bulgular ışığında, ON'nin şiddeti arttıkça eritropotein, Hb, Hct ve eritrosit değerlerinin düştüğü gözlendi. Bu durum ON gelişen diabetli hastalarda ON'nin şiddetinin artmasıyla eritropoetin sekresyonunun azalmasının, renal denervasyonun bir bulgusu olabileceğini düşündürmektedir.
