The effect of urethral catheterisation on serum prostate-specific antigen levels in male patients with acute urinary retention

Abstract
We evaluated the effect of urethral catheterisation on serum prostate-specific antigen (PSA) levels in men with acute urinary retention. A total of 35 men with a mean age 62 years (range 55-92) presented with acute urinary retention at our department between March 1999 and June 2000 were included in the study. Patients were randomly divided into 2 groups; urethral catheterisation was done in 18 cases in the first group (catheterisation group), while suprapubic percutanous cystostomy was performed in 17 patients in the second group (cystostomy group). Serum PSA levels before manipulation and 2, 12 hours and 7 days after treatment were determined. Median serum PSA values before manipulation and 2, 12 hours and 7 days after treatment were 4.5, 4.8, 4.7 and 4.9 ng/ml in the catheterisation group and 3.7, 3.7, 3.7 and 3.9 ng/ml in the cystostomy group, respectively. Change in median PSA values after manipulation was statistically significant in the catheterisation group (p0.05). The change in serum PSA was not clinically important in any of the patients. The results we obtained suggest that urethral catheterisation does not cause a significant alteration in serum PSA in men with acute urinary retention.

Biz, akut üriner retansiyon gelişen hastalarda uygulanan üretral kateterizasyonun serum prostat-spesifik antijen (PSA) düzeyine etkisini araştırdık. Mart 1999 ve Haziran 2000 tarihleri arasında kliniğimize başvuran yaş ortalaması 62 yıl (55-92) olan akut üriner retansiyonlu 35 erkek hasta çalışmaya dahil edildi. Hastalar randomize olarak iki gruba ayrıldı. Birinci gruptaki 18 hastaya üretral kateterizasyon uygulandı (kateterizasyon grubu); kalan 17 hastaya suprapubik perkütan sistostomi tüpü yerleştirildi (sistostomi grubu). Tüm hastaların işlem öncesi, 2, 12 saat ve 7 gün sonraki serum PSA düzeyleri belirlendi. Kateterizasyon grubunda ortanca serum PSA işlemden önce ve 2, 12 saat ve 7 gün sonra sırasıyla 4.5, 4.8, 4.7 ve 4.9 ng/ml, sistostomi grubunda ise 3.7, 3.7, 3.7 ve 3.9 ng/ml idi. işlem sonrası serum PSA düzeyindeki değişim kateterizasyon grubunda istatistiksel olarak önemli (p0.05). PSA düzeyindeki değişim hiç bir hastada klinik öneme sahip değildi. Elde ettiğimiz bulgular akut üriner retansiyonlu erkeklerde üretral kateterizasyon işleminin serum PSA düzeyinde önemli bir değişikliğe neden olmadığını düşündürmektedir.
