The comparison of dexametasone versus metoclop-ramide and ondansetrone on postoperative nausea and vomiting (PONV)

Abstract
PONV is a complication which impairs the patient's comfort. The purpose of this study was to compare the effectiveness of dexametaso-ne versus metoclopramide and ondansetrone to PONV. Fourty four patients aged between 18-70 years old with ASA l-ll who were scheduled for total abdominal hysterectomy and bilateral salpengioooforectomy were randomly divided into four groups. Atropin 0,5 mg and diazepam 10 mg were administered for premedication by intramusculerly. Before the induction of general anaesthesia, metoclopramide 10 mg at Group I, ondansetrone 4 mg at Group II. dexametasone 8 mg at Group III, serum physiologic 2 ml at Group IV were administered. The induction of anaesthesia and intubation were performed with fentanyl 2 mcg/kg, thiopental-Na 4-7 mg/kg and vecuronium 0,1 mg/kg intravenously. Haemodynamic parameters were recorded. The PONV of the patients were evaluated at the end of the operation (zero hour) and 6th, 12th, 24th and 48th hours was recorded. PONV was significantly lower at group I, II, and III at the end of the operation (zero hour) than group IV. There was no difference between the group I, II, III and group IV al the 6th hours. Pleasure of patients of the group I, Il and III significantly higher when compared with group IV at the end of the 48th hours. More than half of the patients were satisfied at group Il and III. It was concluded that at the preventation of PONV, dexametasone is a good alternative to ondansetrone.

Postoperatif bulantı-kusma (POBK) hasta konforunu bozan bir komplikasyondur. Bu çalışmada deksametazonun postoperatif bulantı-kusmayı önlemedeki etkinliğinin metoklopramid ve ondansetronla karşılaştırılması amaçlandı.Total abdominal histerektomi + bilateral salpengo-oforektomi (TAH+BSO) geçirecek ASA l-ll, 18-70 yaş arası 44 hasta 0,5 mg atropin + 10 mg diazepam IM ile premedikasyon yapılarak 4 gruba ayrıldı. Genel anestezi indüksiyonundan önce Grup 1'de 10 mg metoklopramid, Grup ll'de 4 mg ondansetron, Grup lll'te 8 mg dek-sametazon, Grup IV'de 2 mi serum fizyolojik IV uygulandı. 2 mcg/kg fentanil, 4-7 mg/kg tiyopental sodyum, 0.1 mg/kg vekuronyum bremür ile indüksiyon ve entübasyon sağlandı. Hemodinamik parametreler kaydedildi. Hastalar operasyon bitiminde POBK yönünden uyanırken (O. saat), 6., 12., 24. ve 48. saatlerde değerlendirildi. 48 saat sonunda hasta memnuniyeti kaydedildi. POBK'nın 0. saatte kontrol grubuna göre anlamlı olarak önlendiği, Grup I, II, III arasında istatistiksel fark olmadığı, tüm gruplar arasında 6. saatten itibaren fark kalmadığı görüldü.48 saat sonundaki hasta memnuniyeti değerlendirildiğinde, istatistiksel açıdan gruplar arasında fark olmasa bile Grup I, Il ve lll'de kontrol grubuna göre daha yüksek memnuniyet gözlenirken, bu memnuniyetin Grup Il ve lll'de hasta sayılarının yarısından fazla olduğu görüldü. POBK'yı önlemede deksametazonun ondansetrona alternatif olabileceği düşünüldü.
