Dermatological problems developing within two months following coronary By-pass surgery

Abstract
Coronary by-pass operations are performed all over the world. Post-operative complications mainly psychological changes have been investigated. In this report, dermatological complications developing within two months following coronary by-pass surgery were investigated. Thirty-three patients underwent coronary by-pass surgery were enrolled randomly for our study. Patients were examined three times; one week before, one week and two months after the operation. Dermatological findings were recorded unseiectively. The findings recorded in less than three patients were not evaluated. Twelve different findings detected were evaluated in three groups. The first group included the findings existed before the operation and persisting after without any alteration (mycotic infection on foot, cherry angioma, skin tag and varicose veins). The second group contained the findings existed before the operation and progressed afterwards (hyperkeratosis on feet, ichthyosiform xerosis and seborrhea). The last group consisted of the dermatologie conditions occuring after the operation (pretibial edema, ecchymosis, pruritus, decubitus ulcer and hypertophic scar). Findings such as pretibial edema, ecchymosis, pruritus, decubitus ulcer, hypertrophie scar were concluded as directly related with by-pass surgery. On the other hand hyperkeratosis on foot, ichtiosiform xerosis and seborrhea were found to be aggravated after the operation. The rest of the findings were not implicated with by-pass surgery. Dermatological problems related with by-pass surgery should be recognized and primary cautions should be applied for optimal treatment of these patients.

Koroner by-pass ameliyatları tüm dünyada yaygın olarak uygulanmaktadır. Psişik değişiklikler başta olmak üzere neden olabileceği birtakım komplikasyonlar değişik tıp birimlerince araştırılmaktadır. Bu çalışmada da by-pass sonrası ilk iki aylık erken dönemde gelişen dermatolojik komplikasyonlar araştırılmıştır. Çalışmaya koroner by-pass ameliyatı yapılan sıradan 33 hasta alındı. Olguların, ameliyattan bir hafta öncesinde ve bir hafta sonrasında birer kez, 2 ay sonrasında ise bir kez olmak üzere toplam üç kez dermatolojik muayenesi yapıldı. Sınırlama yapılmaksızın tüm dermatolojik bulgular kaydedildi. Üç hastadan daha azında saptanmış olan bulgular değerlendirilmeye alınmadı. Toplam 12 farklı tip bulgu saptandı. Bu bulgular; by-pass öncesinde varolup sonrasında değişmeden devam edenler (ayakta mikotik infeksiyon, "Cherry angioma", "skin tag", varisler), by-pass öncesinde var olup sonrasında progresyon gösterenler (ayaklarda hiperkeratoz, iktiyoziform kserozis, sebore) ve by-pass sonrasında oluşanlar (pretibial ödem, ekimoz, pruritus, dekübitis ülseri, hipertrofik skar) şeklinde 3 grupta değerlendirildi. Bu çalışmada saptanan bulgulardan pretibial ödem, ekimoz, pruritus, dekübitus ülseri ve hipertrofik skar gelişiminin doğrudan by-pass ameliyatına bağlı olarak ortaya çıktığı görülmüştür. Ayaklarda hiperkeratoz, iktiyoziform kserozis ve sebore ise ameliyat sonrasında şiddetlenmiştir. Diğer bulguların ise by-pass ile herhangi bir ilişkisi yoktur. By-pass operasyonları ile ilişkili olabilecek dermatolojik problemlerin bilinmesi ve gerekli tedbirlerin alınmasının hastaların sağaltımı açısından yararlı olacağı düşüncesindeyiz.
