Breast cancer during pregnancy

Abstract

Diagnosis of breast cancer during pregnancy is delayed because of the changes in the physiologic structure of the breasts. For that reason, all the suspicious masses emerging in the breasts during pregnancy must be biopsied for histopathologic examination. We should try to preserve the fetus in patients with the diagnosis of breast cancer. The standard treatment is modified radical mastectomy, although breast conserving surgery can be performed in those patients with the diagnosis made in the 3rd trimester, in which case radiotherapy should be delayed after delivery. Chemotherapeutic agents should not be used in the 1st trimester because of their teratogenic effects. However, specific agents can be safely used in the 2nd and 3rd trimesters. Radiotherapy is contraindicated in all stages of pregnancy. At the same stage, survival is similar of the pregnant and nonpregnant patients with breast cancer. We presented a patient with breast cancer in the 1st trimester and discussed approaches to breast cancer during pregnancy.

Gebelikte memenin fizyolojik yapısının değişmesi meme kanseri tanısında gecikmeye neden olur. Bu nedenle gebelikte ortaya çıkan şüpheli kitlelerde biyopsi ile histopatolojik inceleme gereklidir. Meme kanseri tanısı konulan hastalarda gebelik korunmaya çalışılmalıdır. Standart tedavi modifiye radikal mastektomi olmakla birlikle gebeliğin üçüncü trimesterinde tanı konulan hastalarda meme koruyucu cerrahi uygulanarak radyoterapi doğum sonrasına bırakılabilir, ilk trimesterde teratojenik etkileri nedeniyle kemoterapi ajanları kullanılmaz, ikinci ve 3. trimesterlerde spesifik ajanlar güvenle kullanılabilir. Radyoterapi gebeliğin tüm evrelerinde kontrendikedir. Gebelikteki meme kanserinde sağkalım aynı evredeki gebe olmayan kadınlarla aynıdır. Bu yazıda ilk trimesterde meme kanseri tanısı konmuş bir olgu sunulmuş ve bu konudaki yaklaşımlar tartışılmıştır.
