Abstract

While postoperative nausea and vomiting in craniotomy cases is a cause of patient discomfort, increase of intracranial pressure during the physical activity of vomiting can also cause some deleterious side effects such as deterioration of cerebral perfusion and hemostasis. Medications for the prohylaxis of nausea and vomiting in craniotomy patients should not cause sedation, because postoperative neurologic evaluation is important. The aim of the study is to compare the efficacy on the prophylaxis of postoperative nausea and vomiting and side effects of ondansetron and droperidol in supratentorial craniotomy patients. Following the approval of hospital ethics committee, a total of 36 (n=12) patients between the ages of 15 and 70 in ASA Group l-ll were included in this randomized double blind study. Intramuscular atropin (0,5 mg) and diazepam (10 mg) were given for premedication. Fentanil, thiopental, rokuronium, sevofluran were used for induction. Antiemetics were given intravenously during the closure of the dura, as 5 cc saline in Group P, ondansetron 4 mg in Group O, droperidol 0.625 mg in Group D. Meperidin 1 mg/kg (I.M.) was used for postoperative analgesia. Nausea and vomiting episodes were noted on the 0 min, 30th min, 60th min, 4th hour, 24th hour, and 48th hour. Metoclopromide (10 mg I.V.) was given for the treatment nausea/vomiting. Side effects and complications were noted. Özet

Kraniotomi uygulanan olgularda postoperatif bulantı kusma hasta konforunu bozarken, kafa içi basınç artışı, serebral perfüzyon ve hemostazda bozulma gibi istenmeyen etkiler ortaya çıkarabilir. Bulantı ve kusmanın önlenmesi için kullanılan ilaçlar postoperatif nörolojik takibin önemli olduğu kraniotomi olgularında sedasyon oluşturmamalıdır. Çalışmanın amacı; supratentorial kraniotomi uygulanan hastalarda ondansetron ve droperidol'ün postoperatif bulantı, kusmayı önlemedeki etkinlikleri ve yan etkiler açısından karşılaştırmaktır. Hastane etik komite onayı alınarak ASA l-ll grubunda 15-70 yaş arası 36 (n=12) hasta çalışmaya alındı. Randomize ve çift kör olarak yapılan çalışmada hastalara premedikasyondan sonra fentanil, pentothal, rokuronyum, sevofluran ile genel anestezi uygulandı. Antiemetikler duranın kapatılması sırasında Grup P'ye; 5 cc şalin, Grup O'ya; ondansetron 4mgr, Grup D'ye; droperidol 0.625 mgr İ.V verildi. Postoperatif ; O.dk, 30.dk, 60.dk, 4.st, 8.st, 24.st, 48.st; de; bulantı, kusma epizodları kaydedilerek gerekli ise metoklopromid 10 mgr İ.V uygulandı. Yan etki ve komplikasyonlar kaydedildi. Postoperatif 4-8. saatlerde bulantı grup O ve D'de grup P'ye göre anlamlı olarak az gözlendi (p
