The importance of perinatal history in the etiology of convulsion and neuromotor deficits in childhood

Abstract
The rise factors in the neonatal period have significant effects on the neurologic outcome of the baby. The incidence of seizures and neurologic deficits are seen more frequently in premature and SGA (small for gestational age) infants. Perinatal asphyxia is a frequent cause of seizures and neurologic deficits in term and preterm infants. Seizures and neurologic disorders are encountered more ferquently in the early ages of life, but may be seen in later ages, too. 812 patients who admitted to our pediatric clinic between the years of 2000-2002 with seizure, neuromotor deficit or both of them were included in our study. Hypoxia , SGA and prematurity were present in 183 of the 812 patients. When evaluated according to clinical signs; 93 (67.2%) patients have seizures, 31 (16.9%) have neuromotor deficits, 29 (15.8%) have both of them. When the perinatal history of the patients were evaluated ; 112 (61.2%) have asphyxia, 61 (33.3%) have prematurity, 10 (5.4%) have SGA. The purpose of our study was to identify the importance of perinatal history in the etiology of the seizures and neuromotor deficits in every period of childhood.

Yenidoğan dönemindeki risk faktörleri bebeğin nörolojik gelişimi açısından büyük önem taşır. Erken doğum öyküsü olan veya düşük doğum ağırlıklı yenido-ğanlarda konvülziyon ve nörolojik defisit görülme olasılığı daha fazladır. Perinatal asfiksi (hipoksik-iskemik ensefalopati) hem preterm hem de term bebeklerde sık karşılaşılan bir konvülziyon ve nöromotor gelişim geriliği sebebidir. Tüm bu sorunlar daha çok hayatın erken dönemlerinde görülmekle birlikte geç dönemde de bulgu verebilir. Çalışmamıza hastanemiz Çocuk Sağlığı ve Hastalıkları polikliniğinde 2000-2002 yılları arasında konvülziyon ve/veya nöromotor gelişim geriliği nedeniyle izlenmekte olan değişik yaş grubundaki 812 hasta dahil edildi. Doğum anamnezlerinde hipo-si, düşük doğum ağırlığı, prematürite varlığı olan 183 olgu değerlendirildi. Hastaların başvuru nedenleri irdelendiğinde 93 hastanın (%67.2) konvülziyonla, 31 hastanın (% 16,9) nöromotor gelişim geriliği ile, 29 hastanın (%15.8) konvülziyon ve nöromotor gelişim geriliği ile başvurduğu saptandı. Hastaların doğum anamnezleri değerlendirildiğinde 112 hastada (%61.2) asfiktik doğum öyküsü, 61 hastada (%33.3) prematürite, 10 hastada (%5.4) düşük doğum ağırlığı tespit edildi.Çalışmamızda konvülziyon ve/veya nöromotor gelişim geriliği ile başvuran her yaş grubundan hastada ayrıntılı doğum anamnezinin etyolojiyi ve predispozan faktörleri aydınlatmadaki önemini vurgulamak istedik.
