Approach to patients with multiple trauma

Abstract
Traumas occur quite often in the early ages and account for most of the deaths before the forties. In this article, opinions about the general approach to trauma patients, rather than the treatment of organ systems per se, have been reviewed in the light of the literature. A well-organized management strategy is essential for the cure of trauma patients. The first step is the intervention of specially-trained staff at the place of the accident. Keeping the airway open and providing sufficient breathing have priority. Mobilization of neck, spine, and extremities should be withheld until the fractures are secured. External bleeding should be controlled and the patient should be transferred under oxygen support to the emergency department for appropriate surgical intervention. Instead of lengthy operations, damage control laparatomies after achieving hemostasis and taking precautions for contamination to evaluate the extent of the injury are more proper. By this way, the duration of operations can be reduced, resulting in a decrease in the number of deaths in the operating room.

Travma erken yaşta sık olarak ortaya çıkar ve kırklı yaşlardan önceki ölümlerin çoğundan sorumludur. Bu makalede, travmalı hastalarda organa spesifik tedavilerden ziyade, genel yaklaşımla ilgili fikirler literatür ışığında gözden geçirilmiştir. Travma hastalarının tedavisi için iyi organize edilmiş bir yönetim stratejisi şarttır. Birinci adım, özel eğitimli ekibin kaza yerinde müdahale etmesidir. Hava yolunun açık tutulması ve solunum desteğini sağlamak önceliklidir. Kırıklar emniyete alınıncaya kadar boyun, medulla spinalis ve ekstremite hareketi kontrol altında tutulmalıdır. Dışa olan kanamalar basitçe kontrol edilmeli ve hasta, oksijen desteği altında uygun cerrahi tedavilerin yapılacağı acil servislere nakledilmelidir. Uzun operasyonlar yerine, hemostaz ve kontaminasyon önlenmesi sağlanarak hasar kontrol laparotomileri yapılması daha uygundur. Böylece operasyonların süresi kı-saltılırken, ameliyathanelerdeki ölümlerin sayısı azaltılabilir.
