Effects of clinical functional state on rehabilitation outcome in hemiplegic patients

Abstract
Hemiplegia is one of the most frequent causes of disability and mortality in elderly population. However, validated and comprehensive prognostic models for recovery after stroke have not been developed. The aim of our study was to identify factors affecting the improvement in functional status of hemiplegic patients. 25 hemiplegic patients (17 female, 8 male) were included in this study. The patients were evaluated with respect to gender, age, site of involvement, etiologic factor, impairment in speech and sensation, systemic disease, complication and functional status. In addition, balance, ambulation level and motor functions according to Brunnstrom were assessed at admission and after a two- month rehabilitation. After rehabilitation program significant improvement was found in motor functions and functional status of the patients. Rehabilitation outcomes were not affected by the age, gender, etiology and involved side. Systemic diseases, complications and additional patologies were found to be major prognostic factors for functional independence.
Özet
Hemipleji, yaşlı nüfusta en sık karşılaşılan sakatlık ve ölüm nedenlerinden biridir. Bununla birlikte, iyileşme için geçerli ve kapsamlı bir prognostik model geliştirilememiştir. Çalışmamızın amacı, hemiplejik hastalarda fonksiyonel durumdaki iyileşmeyi etkileyen faktörleri belirlemekti. On yedisi kadın, 8'i erkek, toplam 25 subakut dönem hemiplejik hasta çalışmaya dahil edildi. Hastalar yaş, cinsiyet, tutulan taraf, etiyolojik faktör, konuşma ve algı bozukluğu, denge, ambulasyon, sistemik hastalık, komplikasyon ve fonksiyonel durumlarına göre değerlendirildi. Hastaların tedavi öncesindeki ve 2 aylık rehabilitasyon sonrasındaki dengesi, ambulasyon seviyesi ve Brunnstrom'a göre motor fonksiyonları belirlendi. Rehabilitasyon programı sonunda, hastaların motor fonksiyonlarında ve fonksiyonel durumlarında anlamlı iyileşme tespit edildi. Rehabilitasyon sonuçları yaş, cinsiyet, tutulan taraf ve etiyolojiden etkilenmiyordu. Sistemik hastalıklar, komplikasyonlar ve ek patolojiler fonksiyonel bağımsızlık için majör prognostik faktörler olarak tespit edildi.
