Comparison of Intraarticular Hyaluronic acid and short wave diathermy on the treatment of knee osteoarthritis

Abstract
This study has been conducted on 40 patients who consulted to Haydarpaşa Numune Hospital, Physical Medicine and Rehabilitation Clinic with the complaint of knee pain, to investigate and compare the effectivenesses of the intraarticular sodium hyaluronate (HA) and short wave diathermy which stand as two high-valued treatment options in knee osteoarthritis (OA). In this single-blind, prospective and randomized study, the 40 patients were divided into two groups. While 20 patients in one group were received with 2 ml intrarticular 1 % HA once a week for a total of 3, shortwave diathermi was applied to the other group 5 times a week for a total of 15. In addition, both groups were informed of the protective precautions and they were made to practice isometric knee exercises after a hot-pack application of 20 minutes. The patients are evaluated in terms of VAS, WOMAC OA index, Lequesne index, range of joint movement measurement and analgesic usage need, for a total of 6 times; one in the beginning and the others on the 1st, 2nd, 3rd, 7th weeks and on the 3rd month, and the groups were compared. In the end of the study, in the patients of the HA group a significant improvement on all parameters was achieved at the end of the 1 st week, which stayed steady until the 3rd month. On the other hand, in the patients of the short wave diathermy group, altho this improvement was prominent in the first two weeks, it is observed that the average point of the VAS and WOMAC Osteorthritis Index were lower than in the HA group. In conclusion, intraarticular HA and short wave diathermy were found to be effective and reliable methods in the patients with knee OA. Özet
Diz osteoartritinde (0A) iki değerli tedavi seçeneği olan intraartiküler sodyum hyalurinat (HA) ve kısa dalga diyaterminin (KDD) etkinliklerini araştırmak ve karşılaştırmak amacıyla yapılan bu çalışma, Haydarpaşa Numune Hastanesi «Fizik Tedavi ve Rehabilitasyon Kliniğine diz ağrısı şikayeti ile başvuran 40 hasta üzerinde yapılmıştır. Prospektif, randomize, tek kör olarak gerçekleştirilen çalışmada 40 hasta iki gruba ayrılarak 20 hastaya haftada bir kez, toplamda 3 kez 2ml intrartiküler %1 HA, diğer 20 hastaya haftada 5 seans, toplamda 15 seans kısa dalga diatermi uygulandı. Ayrıca her iki gruptaki hastalara koruyucu önlemler hakkında bilgi verilerek 20 dakika sıcak paket uygulaması sonrasında izometrik diz egzersizleri yapmaları sağlandı. Hastalar başlangıç, 1,2,3,7. hafta ve 3. ayda, toplamda 6 kez VAS, W0MAC OA indeksi, Lasquesne indeksi, eklem hareket açıklığı ölçümü ve analjezik kullanım ihtiyacı açısından değerlendirildi ve gruplar birbiriyle karşılaştırıldı. Çalışma sonunda HA grubundaki hastalarda 1. hafta sonunda tüm parametrelerde anlamlı düzelme sağlanmış olup 3. aya kadar aynı seviyede devam etmiştir. Diğer yandan KDD grubundaki hastalarda bu düzelme ilk iki haftalık dönemde belirgin olup 3. aydaki VAS ve WOMAC Osteoartrit İndeksi ortalama puanlarının HA grubundan daha düşük olduğu gözlenmiştir. Sonuç olarak, OA'li hastalarda intraartiküler HA ve KDD tedavisinin etkin ve güvenilir yöntemler olduğu tespit edilmiştir.
