Surgical approach to colorectal cancer

Abstract
We investigated 41 patients operated on for colo-rectal cancer in the 5th General Surgery department of Haydarpaşa Numune Education and Research Hospital between December 2003 and December 2006, retrospectively. Mean age was 62.40 years. The most common complaints at presentation were abdominal pain (39.02%) and weight loss (36.58%). Masses were located in the left colon and rectum in 78.05% of the patients. The most common stage was Stage C2 of Astler Coller with an incidence of 58.55%. Moderately differentiated adenocarcinoma (58.54%) was the most common tumor pathology. In the early postoperative period we reported wound infection in 4 (9.75%) patients and anastamotic leak in 1 (%2.43). Patients were followed once every 3-month-period in the first 2 years and once every 6-month-period in the other 2 years with physical examination, determination of serum carcinoemb-ryonic antigen (CEA) and tests for liver function,ectosigmoidoscopy if rectal resection was performed while chest X-ray and abdominal ultraso-nography were performed once every year. We reported hepatic metastasis in 4 (9.75%) patients, recurrence at the site of anastamosis in 1 (2.43%) and frozen pelvis in 1 (2.43%) at postoperative follow-up. Histopathologic determination of the specimen showed Stage C2 in 24 patients and Stage D in 1 patient. These patients were administered 6 sessions of chemotherapy with 5-Flourouracil (5FU) and folinic acid. Among patients with rectum cancer in 7 patients with Stage C2 and in 1 with Stage B2 tumor radiotherapy was added to the systemic therapy protocol.

Haydarpaşa Numune Eğitim ve Araştırma Hastanesi ö.Genel Cerrahi Kliniğinde Aralık 2003-Aralık 2006 tarihleri arasında kolorektal kanser nedeniyle öpere edilen 41 hasta retrospektif olarak incelendi. Ortalama yaş 62.40'idi. En sık başvuru şikayeti karın ağrısı ( %39.02) ve kilo kaybıydı (% 36.58 ). Kitlelerin %78.05'i sol kolon ve rektum yerleşimliydi. Aster Coller evrelemesine göre Evre C2 %58.55 oranında en fazla görülen evreydi. Tümör patolojilerinde en fazla orta derecede diferan-siye adenokarsinom görüldü ( %58.54). Postope-ratif dönemde 4 (%9.75) hastada yara yeri infeksi-yonu , 1 (%2.43 ) hastada anastomoz kaçağı görüldü. Hastalar İlk 2 yıl 3 ayda bir, sonraki 2 yıl her 6 ayda bir fizik muayene, karsinoembriyonik antijen (CEA), karaciğer fonksiyon testleri, rektum re-zeksiyonu uygulanmışsa rektosigmoidoskopi, yılda bir akciğer grafisi ve batın ultrasonografisi (USG) takip programına alındı. Postoperatif takiplerde 4 (%9.75 ) hastada karaciğer metastazı , 1(%2.43 ) hastada anastomoz hattında nüks ,1(%2.43) hastada frozen pelvis görüldü. Postoperatif histopatolojik değerlendirme sonucu 24 hasta Evre C2 , 1 hastada Evre D 'idi. Bu hastalar 5 Fluorourasil (5FU ) ve Folinik asit ( FA ) ile 6 kür kemoterapi programına alındı. 7 rektum tümörlü Evre C2 hastanın kemoterapi programına radyoterapi eklendi. Rektum yerleşimli Evre B2 'li bir hasta aynı kemoterapi ve radyoterapi programına alındı.
