Determinationof lymphocyte cross-match test results before kidney transplantation retrospectively: Microlymphocytotoxicity assay

Abstract
Aim: To perform lymphocyte cross-match test between patient and donor before kidney transplantation for prevention of hyperacute rejection (HAR) and to evaluate the pretransplant lymphocyte cross-match test results between January 2007 and December 2008 retrospectively. Material and Method: Lymphocyte cross-match test has been performed by microlymphocytotocity assay; in other words complement-dependent cytotoxicity test (CDC) serologically. Sera from eighty-one patients have been included in this study. Man to woman ratio is 43/38. Totally 146 CDC test have been studied. Ratio of cadaveric to live donor is 15/131. Results: Totally 146 CDC tests have been applied to 81 included patients. According to test results; 11 were positive and 135 were negative among all pretransplant cytotoxic cross-match tests. Positive cross-match test ratio was found to be 7.5% in pretransplant period of all included tests. Conclusion: CDC test is still accepted as a standard and classical technique in determination of donor-specific anti-HLA antibodies before kidney transplantation for prevention of HAR.

Amaç: Hiperakut rejeksiyonun (HAR) önlenmesi için böbrek naklinden önce hasta ile verici arasında lenfosit cross-match test çalışılması ve Ocak 2007 ile Aralık 2008 tarihleri arasında yapılan pretransplant lenfosit crossmatch test sonuçlarının retrospektif değerlendirilmesi. Materyal-Metod: Lenfosit cross-match testi, serolojik bir yöntem olan mikrolenfositotoksisite yöntemi; diğer adıyla komplemana bağımlı sitotoksisite testi (CDC) ile gerçekleştirilmiştir. Çalışmaya 81 hasta materyali dahil edilmiştir. Erkek/ Kadın oranı (43/38) dir. Toplam 146 CDC test çalışılmıştır. Kadavra/Canlı donör oranı (15/131) dır. Bulgular: Çalışmamıza katılan 81 hastaya toplam 146 test uygulanmıştır.Pretransplant sitotoksik cross-match test sonuçlarından 11 pozitif ve 135 negatif bulunmuştur. Pretransplant dönemde çalışılan toplam testlerde %7.53 cross-match pozitişik tespit edilmiştir. Sonuç: CDC testi HAR önlenmesinde donor-spesifik anti- HLA antikorların tayin edilmesinde hala klasik ve geçerli bir teknik olarak kabul edilmektedir.
