Skin test to anesthetics in asthmatic patients

Abstract

Allergic reactions due to general anesthetics are rarely reported, but may involve life-threatening reactions including anaphylaxis. Anesthetics must be carefully applied especially to the patients who have experienced allergic reactions during surgery or the ones who have hypersensitivity reactions to certain drugs. The aim of this study was to evaluate the potential risk of asthma for hypersensitivity reactions due to general anaesthetics. Patients (n= 155) who were sent to our outpatient clinic to be tested for general anaesthetics and healthycontrols (n= 19) were included in the study. Asthma, previous allergic reactions during surgery, drug hypersensitivity, and other risky situations were questioned in details. There were 42 asthmatic patients (female 35; mean age 42±14 years old). The acquired anesthetic materials including propophol, etomidate, atracurium, cisatracurium, rocuronium, vecuronium, mivacurium, pethidine, phentanyl, neostigmin, atropin were applied on the forearm of the subjects according to the previously described protocol. Skin prick tests in 1/10 dilution were firstly applied, and when found negative intradermal tests in 1/1000 dilution were done. In patients who previously experienced allergic reactions due to the anaesthetics were applied further dilutions of anesthetics including 1/100 and 1/10 in intradermal tests respectively unless positive test results were reached. Skin tests were regarded as positive when wheal diameter was at least 3 mm greater than the negative control. The positive test results were found as follows: Propofol 40.4%, pethidine 48.3%, atracurium 17.2%, vecuronium 10.4 %, latex 1.5 % in all patients; propofol 44.7%, atracurium 7.1%, neostigmin 7.1 %,vecuronium 5%, etomidate 4.2% in asthmatics. Positive test results were not significantly different in asthmatics, other patients and the controls (p>0.05). Although high positive rates to propofol and pethidine were found in all groups, this situation was thought to be related to the direct histamine release effect of these agents. In conclusion patients with asthma may not be suspected to have a higher risk for allergic reactions due to general anesthetics. However general anaesthetics showing high positive rates may be used with precaution in patients posing potential risk.

Genel anestetiklerle ortaya çıkan allerjik reaksiyonlar nadir görülmekle birlikte ölüme kadar gidebilen reaksiyonlara yol açabilirler. Daha önceki bir ameliyatta reaksiyon öyküsü olan veya ilaç allerjisi, astım gibi risk taşıyan bazı hastalarda genel anestetik seçiminde dikkatli olunmalı dır. Bu amaçla çalışmamızda polikliniğimize genel anestetiklere karşı allerji deri testi yapılmak üzere gönderilen 155 olgu ile 19 sağlıklı kişiden oluşan kontrol grubunu karşılaştırarak astımın muhtemel risk faktörü olup olmadığını araştırmak istedik. Tüm olguların 42’sinde astım nedeni ile test istenmişti. Bunların 35’i kadın, 7’si erkek, yaş ortalaması 42±14’tü. Propofol, etomidate, atracurium, cisatracurium, rocuronium, vecuronium, mivacrium, pethidin, fentanyl, neostigmin, atropin (hangi anestetik istendi ise) ile ön kol içyüzüne 1/10 sulandırma ile prik test, bu test negatif ise 1/1000 sulandırma ile intradermal test, testi negatif olup daha önce anesteziye bağlı sorun yaşayan olgularda sırası ile 1/100 ve 1/10 sulandırma ile intradermal testler yapıldı. Negatif kontrole göre 3 mm ve daha fazla kabarıklık ve kızarıklık pozitif reaksiyon olarak değerlendirildi. Tüm hastalar içinde en sık pozitif reaksiyon oranları: Propofol %40.4, pethidin %48.3, atracurium %17.2, vecuronium %10.4 idi. Latex deri testi pozitişiği % 2.1 idi. Astımlı olgularda ise propofol % 44.7, atracurium %7.1, neostigmin % 7.1, vecuronium % 5, etomidate %4.2 olguda pozitif idi. Cilt testi pozitişiği bakımından astımlı hastalarla diğer hastalar ve sağlıklı kontrol grubu arasında anlamlı bir farklılık bulunmadı (p>0.05). Propofol ve pethidine karşı pozitif deri testi oranı oldukça yüksek olmakla birlikte bu oranı n, propofol ve pethidinin direkt histamin salgılatıcı özelliği ile ilgili olduğu düşünüldü. Sonuç olarak; astımlı hastaların genel anestetiklere duyarlı lık bakımından daha yüksek bir risk altında olduğunu düşündürecek bir bulgu elde edilememiştir. Ancak yine de muhtemel risk grubundaki olgularda pozitişik oranı yüksek anestetiklerin dikkatle kullanılması uygun olabilir.
