Retrospective evaluation of the patients who were hospitalized with the prediagnosis of mushroom intoxication in Haydarpşa Numune Training and Research Hospital between 2005 and 2006

Abstract
In Turkey because of the appropriate ecological conditions many types of poisonous mushroom species grow. As in all over the world, amanita species are the most toxic one among the mushroom species in our country. In this study 84 patients who were hospitalized in the clinics of Internal Medicine and Pediatry of our hospital between 2005-2006 were evaluated retrospectively. Maximum applications to our hospital were in October (%86.4). Most of the cases were from Asian-side of ‹stanbul (%92.8). The average period of time between the exposure to the toxin and the beginning of the complaints was about 3.65 hours in our patients. It proves that most of mushroom intoxication cases that applied to our hospital were mushroom species which show short latent period. Mushroom intoxications having short latent period have better prognosis. Most common complaint was nausea(%91.4). Among the 84 patients , ALT (Alanine aminotransferase) was high in 3 patients, AST (Aspartate transferase) in 13 patients, PT (Prothrombin time)-INR (International normalized ratio) in 15 patients, BUN (blood urea nitrogen) in 4 patients and creatine in 1 patient at the first moment of application. During the clinical follow-ups, improvement of the laboratory findings were seen in all patients. Nine patients which show long latent period underwent hemoperfusion and detoxification and 3 patients died.In mushroom intoxications, the basis of treatments are preventing patients from the effects of the toxin and increasing the clearance of toxins from body. Incidence of permanent organ damage and mortality can be decreased with early diagnosis and treatment. Preventive medicine studies especially education of the population will make a great contributions to decrease of mortality and morbidity.

Türkiye’de uygun ekolojik koşullar nedeni ile birçok zehirli mantar türü yetişmektedir. Tüm dünyada olduğu gibi ülkemizde de en toksik etkiler gösteren mantarlar amanita türleridir. Bu çalişmada 2005-2006 tarihlerinde Haydarpaşa Numune Eğitim ve Araştirma Hastanesi Dahiliye ve Pediatri Kliniklerine yatirilan toplam 84 hasta retrospektif olarak değerlendirildi. Hastaneye en sik başvuru ekim ayi içinde olmuştur (%86.4). Olgularin büyük kismi istanbul Anadolu yakasindan gelmiştir (%92.8). Hastalarin mantar yedikten sonra şikayetlerinin başlamasina kadar geçen ortalama süre 3.65 saattir. Bda hastanemize başvuran mantar zehirlenmesi olgularinin büyük çoğunluğunun kisa latent süre gösteren man- tar türleri ile olduğunu ortaya koymaktadir. Kisa latent süreli mantar zehirlenmeleri daha iyi prognoza sahiptir. En sik görülen şikayet bulanti (%91.4)‘dir. 84 hasta içerisinde hastaneye ilk başvuru aninda 3 hastada alanin aminotransferaz (ALT), 13 hastada aspartat aminotransferaz (AST), 15 hastada protrombin zamani (INR), 4 hastada BUN (kan üre nitrojeni) ve 1 hastada kreatinin dü- zeyleri yüksek olarak bulundu. Klinik takipler sirasinda tüm hastalarin laboratuvar değerlerinde düzelme görüldü. Uzun latent süreli 9 hastaya hemoperfüzyon ile detoksifikasyon uygulandi, 3 hasta kaybedildi. Mantar zehirlenmesinde toksinin etkilerini önlemek, klirensini arttirmak tedavinin temeli-ni oluşturmaktadir. Erken tani ve tedavi programinin erken dönemden itibaren uygulanmasi ile kalici organ hasari insidansi vemortalite azalmaktadir. Koruyucu hekimlik çalişmalari başta eğitim olmak üzere mortalite ve morbiditenin azaltilmasinda büyük katki sağlayacaktir.
