ANESTHESİA MANAGEMENT OF A CASE WİTH MELAS SYNDROME

Abstract

MELAS syndrome is a disease which delevops as a result of point mutation on mitochondrial level and characterized with mitochondrial myopathy, lactic acidosis and stroke like episodes. Surgical and anesthesia procedures on MELAS syndrome diagnosed patients exhibites special featu res since these patients present tendency towards clinical situations such as hypothermia, metabolic acidosis and malign hyperthermia. Syndrome frequently co-exists with diabetes mellitus and cardiac conduction anomalies and this makes the situation more complicated. In this paper, anesthesia management for elective thyroidectomy operation in a MELAS syndrome case with co-existing diabetes mellitus, hypertrophic cardiomyopathy diagnoses is presented and anesthetic approach is discussed.

Özet

MELAS sendromu mitokondrial miyopati, ensefalopati, laktik asidoz ve stroke benzeri epizodlarla karakterize olan mitokondriyal düzeyde nokta mutasyonu sonucunda gelişen bir hastalıktır. MELAS sendromu tanısı olan hastalarda cerrahi ve anestezi uygulamaları hastaların hipotermi, metabolik asidoz ve malign hipertermi gibi klinik durumlara yatkın olmaları nedeniyle özellik oluşturur. Sendromun tablosuna sıklıkla diyabetus mellitus ve kardiyak iletim anomalileri eşlik eder ve durumu daha da karmaşık hale getirir. Bu yazıda elektif tiroidektomi operasyonu geçiren ve diyabetus mellitus, hipertrofik kardiyomiyopati tanılarının eşlik ettiği MELAS sendromlu bir olguda anestezi yönetimi sunulmuş ve anestezik yaklaşım tartışılmıştır.
