Morphological assessment of bomding mechanism to groundand unground enamel of fluorosed and normal teeth: A Fe-SEM study

Abstract
Aim: It is important to bond to fluorosed enamel with acid-resistant outer hypermineralized layer for the current adhesive systems. The aim of this study is to morphologically assess the etching efficacy and resinenamel interface of 37.5% phosphoric acid etchant and a self-etch primer on ground and unground enamel in fluorosed and normal teeth by a Fe-SEM. Materials and Methods: Buccal/lingual, ground and unground enamel from fluorosed (TFI 5) and normal (TFI 0) molar teeth were etched with either 37.5% phosphoric acid gel (Kerr Gel Etchant) or self-etch primer (Clearfil Protect Bond Primer, Kuraray) according to the manufacturers’ instructions. The specimens were rinsed off for the examination of etching patterns. Furthermore, for examination of the resin-enamel interface, Optibond FL and Clearfil Protect Bond were applied to the specimens, covered with a low viscosity resin and then sectioned perpendicularly to the interface. All the specimens were processed for Fe-SEM using a common specimenprocessing protocol. Results: Microporosities based on interprismatic areas and apatite crystals in prisms were observed for the phosphoric acid etchant, whereas the self-etch primer resulted in an etching pattern characterized by ranging from nearly absent demineralization on prisms, especially for the unground fluorosed enamel surfaces, to shallow demineralization on the enamel surface. When the adhesive interface between the prepared and unprepared enamel and the resin wasexamined, the etched zone created by Optibond FL was deeper than that created by Clearfil Protect Bond in both fluorosed and normal teeth. Conclusion: A well-defined etching pattern was not observed for the unground enamel in fluorosed teeth for the self-etch primer tested. Preparation of enamel improved the etching efficacy in fluorosed teeth.

Amaç: Günümüzdeki adeziv sistemlerin, dış yüzeyde aside dirençli hipermineralize tabakanın bulunduğu florozisli dişlerde, mine dokusuna bağlanabilmeleri önemlidir. Bu çalışmanın amacı, prepare edilmiş ve edilmemiş florozisli ve normal mine yüzeylerinde %37.5’luk fosforik asit ve bir self-etch primer ile yapılan asitlemenin etkinliğinin ve rezin-mine arayüzünün bir Fe-SEM ile morfolojik olarak değerlendirilmesidir. Gereç ve Yöntem: Prepare edilen ve prepare edilmeden bırakılan florozisli (TFI 5) ve normal (TFI 0) molar dişlerin bukkal/lingual mine yüzeyleri %37.5’luk bir fosforik asit jeli (Kerr Gel Etchant) ve bir self-etch primer (Clearfil Protect Bond Primer, Kuraray) ile üretici firmaların önerilerine göre asitlendi. Örnekler, asitleme modelinin incelenebilmesi için yıkandı. Ayrıca rezin-mine arayüzünün incelenmesi için Optibond FL ve Clearfil Protect Bond uygulanan örnekler düşük viskoziteli bir rezin ile kaplanarak arayüze dik olarak kesildi. Tüm örnekler elektron mikroskop için bilinen örnek hazırlama teknikleri kullanılarak Fe-SEM’de incelendi. Bulgular: Asitleme modelinin, fosforik asit ile elde edilen prizmalar arasında ve prizmalardaki kristallerde gözlenen mikropörözitelerden, self-etch primer ile elde edilen yüzeysel bir demineralizasyona ve özellikle prepare edilmemiş florozisli yüzeylerdeki hiçbir çözülmenin olmadığı prizmalara kadar değişiklik gösterdiği saptandı. Prepare edilmiş ve edilmemiş mine ile adeziv rezin arasındaki yüzey incelendiğinde, hem florozisli hem de normal dişlerde, Optibond FLile elde edilen asitlenmiş alanın Clearfil Protect Bond ile elde edilen asitlenmiş alandan daha derin olduğu gözlendi. Sonuç: Minede preparasyonun yapılmadığı florozisli dişlerde test edilen self-etch primer ile belirgin bir asitleme modeli elde edilmedi. Florozisli dişlerde minede yapılan preparasyon asitlemenin etkinliğini artırdı.
