Sump syndrome and its endoscopic treatment

Abstract

Side-to-side choledochoduodenostomy is a safe and effective surgical technique to improve biliary drainage in selected patients. But debris, stones or infected bile accumulates in the segment of common bile duct between the anostomosis and the ampulla of Vater and may develop symptoms. These symptoms are due to cholangitis, pancreatitis and biliary obstruction. This uncommon complication is known as the "sump syndrome".' For along time, surgical therapy was the only possible treatment, but in recent years, endoscopic interventions have been advocated as well. We report here two sump cases which have been treated by endoscopic way.

Özet

Yanyana yapılan koledokoduodenostomi, seçilmiş hastalarda safra drenajını düzelten etkin ve güvenilir bir işlemdir. Ancak, anastomoz ile ampulla Vateri arasında kalan koledok segmentinde debris (= çökel¬ti), taş ve infekte safra birikerek semptomlara yol açabilir. Semptomlar kolanjit, pankreatit ve biliyer obstrüksiyon nedenleriyledir. Seyrek görülen bu komplikasyonlar "sump sendromu" olarak bilinir. Ön¬celeri, bu sendromda cerrahi girişim tek tedavi seçe¬neği iken, günümüzde terapötik endoskopi yöntemiy¬le de başarılı sonuçlar elde edilmektedir. Burada endoskopik yolla tedavi edilen iki sump sendromu ol¬gusu sunulmaktadır.
