The role of coulter counter and phase contrast microscope in estimating the cause of hematuria

Abstract
Eigthy-two hématurie patients (30 glomerular and 52 nonglomerular) were included into this study to determine the role of Coulter counter and phase contrast microscope in estimating the origin of hematuria and the research was performed by means of phase contrast microscope (PCM) and Coulter counter to assess the urinary red cell morphology. Urinary red cell morphology was examined by PCM and urinary red cells mean corpuscular volume (UMCV) was determined by Coulter counter for all the patients. In 54 cases, red cells volume distributions curves (RCVDC) were recorded. Urinary pH and spesific gravity by dipstick and urinary osmolality of 41 pati-ents were measured. The source of hematuria in 26 out of 29 glomerular patients and 46 out of 52 nonglomerular patients were accurately assessed by PCM. Meanwhile, the source of hematuria was correctly determined in 28 out of 30 glomerular pati-ents and 46 out of 52 nonglomerular patients by UMCV; in 10 out of 11 glomerular patients and 39 out of 43 nonglomerular patients by RCVDC. No correlation among the results of PCM and Coulter counter with urinary pH, osmolality and spesific gravity was found. The specificity and sensitivity of urinary red cell norphology examined by PCM in defining glomerular hematuria were 88,5 % and 89,7 %, respectively. The specificity and sensitivity of UMCV were 88,5 %, 93,3 % respectively. Finally, RCVDC had a specificity of 90,7 % and a sensitivity of 90,9 %. Consequently; in the estimation of the source of hematuria, PCM and Coulter counter were accepted as being noninvasive and cost effective methods. Also, among the steps of the evaluation of hematuria, the usage of PCM and Coulter counter initially would prevent the patients from many unnecessary diagnostic procedures.

Hematüıi nedenlerini araştırmada, üriner eritrosit morfolojisi ve büyüklüğünün faz kontrast mikroskop (FKM) ve otomatik tam kan sayıcı (OTKS) kullanarak belirlenmesinin değerinin araştırıldığı bu çalışmaya 30'u glomerüler, 52'si nonglomerüler hematürili 82 hasta alındı. Hastaların tümünde FKM ile idrar erit-rosit morfolojisi ve OTKS ile idrar eritrosit büyüklüğü (ortalama eritrosit hacmi (lOEH) ve 54 olguda idrar eritrositlerinin hacim dağılım eğrileri (EHDE) saptandı. Hastaların 41'inde idrar ozmolalitesi ve dipstik ile idrar pH ve dansitesi saptandı. FKM ile glomerüler hematürili 29 hastanın 26'sında, nonglomerüler hematürili 52 hastanın 46'sında hematürinin kaynağı doğru olarak belirlendi. OTKS ile lOEH'ne göre 30 glomerüler hematürili hastanın 28'inde ve 52 nonglomerüler hematürili hastanın 46'sında, EHDE'lerine göre 11 glomerüler hematürili hastanın 10'unda 43 nonglomerüler hematürili olgunun 39'unda hematürinin kaynağı doğru olarak tahmin edildi. FKM ve OTKS sonuçları ile idrar pH, ozmola-lite ve dansitesi arasında ilişki bulunmadı. Glomerüler hematüriyi belirlemede; FKM ile eritrosit morfolojisini değerlendirmenin; spesifisitesi % 88,5 ve sensitivitesi % 89,7, lOEH'nin; spesifisitesi % 88,5 ve sensitivitesi % 93,3 ve EHDE'nin; spesifisitesi % 90,7 ve sensitivi-tesi % 90,9 bulundu. Sonuçta; FKM ve OTKS'nin hematüri kaynağının tahmininde invaziv olmayan ve ucuz yöntemler olarak yararlı oldukları görüldü. Hematürinin değerlendirme basamakları arasında da başlangıçta kullanımları, hastaları bir çok gereksiz tanısal girişimlerden koruyabilir.
