The importance of the preoperative CRP in diagnosis of acute appendicitis

Abstract
C-Reactive protein (CRP) is an acute phase protein. Plasma concentration of CRP rises significantly in case of inflammation and tissue destruction. Between 1995 and 1997 plasma CRP levels weremeasured in 76 patients, then their preoperative diagnosis were made as acute appendicitis according to clinical evaluation and leucocyte count. 28 patients with CRP level less than 6 mg/lt and 48 patients with CRP level more than 6 mg/lt were classified into Group-l and II. Only 4 patients in Group-l and 43 patients in Group-ll were diagnosed as acute appendicitis with the histopathologic examination of appendectomy material in the postoperative period. C-reaktif Protein (CRP) bir akut faz proteinidir. CRP serum konsantarsoyonu inflamasyon ve doku yıkımı meydana geldiğinde anlamlı düzeyde yükselmektedir. Klinik durum ve lökosit değerlerine göre akut apandisit düşünülen 76 olguya 1995-1997 yılları arasında, appendektomi ameliyatı yapılmıştır. CRP 6 mg/l altında olan olgular Grup-l (n=28), üstünde olanlar ise Grup-ll (n=48) de değerlendirilmiştir. Ameliyat sonrası dönemde appendektomi materyalinin histopatolojik değerlendirilmesinde Grup-l'de sadece 4 olguda, Grup-ll'de ise olguların 43'ünde akut apandisit tespit edilmiştir. CRP düzeyi negatif olarak belirlenen olgularda, normal appendiks ve akut apandisit görülme oranları karşılaştırıldığında fark anlamlı bulunmuştur (p
