High dose chemotherapy with peripheral blood stem cell support in limited small cell lung cancer

Abstract

Although limited small cell lung cancer (SCLC) is chemosensitive, rarely can it be curable. Highly significant dose-response relation reported at clinical studies in these patients shows that overall survival can be increased by high dose chemotherapy. So a study was planned related to high dose chemotherapy with peripheral blood stem cell support followed by thoracal and prophylactic cranial radiotherapy in SCLC; and the first case to whom this protocol was applied was reported in this article. 6 courses of chemotherapy (cisplatin, etoposide, ifosfamid) and thoracal radiotherapy in dose of 300 cGy x 10 were given to 54 years old male with SCLC limited to right chest in December 1995. After getting complete response to that therapy, the case was admitted to high dose chemotherapy and peripheral blood stem cell trans-plantation protocol. 6,85 x $10^8$/kg mononuclear cells and 10,1 x $10^6$/kg CD34 (+) cells were collected to-tally by progenitor cell mobilisation procedure with 10 $mu$gr/kg/day G-CSF. ICE chemotherapy (ifosfamide 12 gr/$m^2$), carboplatin 1,2 gr/$m^2$ and etoposide 1,2 gr/$m^2$; in 6 days) was administered. Neütrophil (> 500/ $mm^3$) and platelet (>50.000/mnP) engraftments were achieved on $13^th$ and $19^th$ days of reinfusuon, respectively. We did not observe any serious toxicity findings except nausea-vomiting (grade 2), mucositis (grade 1) andalopesia (grade 4). 4 erythrocyte and 4 platelet transfusions totally were given and prophylactic cranial radiotherapy in dose of 300 cGy x 10 was applied to the patient in posttransplant 2 nd month. Özet

Sınırlı küçük hücreli akciğer kanseri (KHAK) kemote-rapiye duyarlı bir tümör olmasına rağmen nadiren kür sağlanabilmektedir. Bu hastalarda yapılan çalışmalarda gösterilen çok anlamlı bir doz-cevap ilişkisi, kemoterapi dozunun artırılması ile yaşam süresinde uzama elde edilebileceğini düşündürmektedir. Bura-da KHAK'li olgularda yeni olarak uygulamaya başladığımız peıiferal kan kök hücre desteği ile yüksek doz kemoterapi, takiben torakal ve profilaktik kranial ışınlama tedavi modalitesi ve bu tedavinin uygulandı-ğı ilk olgu sunulmuştur. 54 yaşında, erkek olguya Aralık 1995 tarihinde sağ hemitoraksta sınırlı KHAK tanısı ile 6 kür kemoterapi (Cisplatin, etoposid, ifosfamid) uygulandı ve ardından 300 cGy x 10 dozunda torasik ışınlama yapıldı. Bu tedavüle tam yanıt alınan olgu daha sonra yüksek doz kemoterapi ve periferal kan kök hücre transplantasyonu protokolüne alındı. 10 $mu$gr/kg/gün dozunda G-CSF ile yapılan progenitor hücre mobilizasyonu ile toplam 6.85 x $10^8$/kg mononükleer hücre ve 10.1 x $10^6$ / kg $CD_{34}$+ hücre elde edildi. Hazırlama rejimi oarak ICE kemoterapisi (ifosfamid 12 gr/m2, karboplatin 1.2 gr/$m^2$ ve etoposid 1.2 gr/$m^2$; 6 güne bölünerek) verildi. Periferal kan kök hücre reinfüzyonundan 13 gün sonra nötrofil engraftmanı (> 500/$mm^3$), 19 gün sonra trombosit engraftmanı (> 50.000/$mm^3$) sağlandı. Olguda bulantı-kusma (Grade 2),mukositis (Grade 1) ve alopesi (Grade 4) dışında ciddi bir toksisite bulgusu gözlenmedi. Erken dönemde toplam 4 eritrosit, 4 trombosit transfüzyonu uygulanan olguya posttransplant 2. ayda tüm beyin profilaksisi için 300 cGy x 10 dozunda kranial ışınlama uygulandı. Posttransplant 75. günde nüks tespit edilip paclitaxel (135 mg/$mg^2$) ve farma-rubisin (60 mg/m2) başlandı. Halen posttransplant 110. günde olup, olgunun kemoterapisi devam etmektedir.
