Diagnostic value of polymerase chain reaction in tuberculous pleurisy

Abstract

The purpose of our study was to detect the presence of M. tuberculosis bacilli in pleural fluid by means of polymerase chain réaction (PCR) technique, and evaluate the diagnostic merit of this modality in tuberculous pleuritis (TP). We sought with PCR the IS6110 region specific for M. tuberculosis DNA and showed the products with hybridization-electrophoresis-gel-ultraviolet method. We performed a comparison between PCR results and the outcomes of the conventional tests, namely pleural fluid smear/culture and pleural biopsy histopathology with respect to the clinical diagnosis, and calculated statistical parameters on this basis. The values for the study group, certain TP subgroup and suspect TP subgroup, respectively, were as follows: sensitivity; 52.8%, 72.4% and 29.1%; specificity; 95%, 95% and 95%; positive predictive value; 96.5%, 95.4% and 87.5%; negative predictive value; 43.1%, 70.3% and 52.7%; total accuracy; 64.3%, 81.6% and 59.0%. PCR has a sensitivity greater than pleural fluid smear/culture and pleural biopsy histopatology in the diagnosis of pleural tuberculosis. By utilizing the appropriate DNA primers, it is possible to detect M. tuberculosis DNA within 24 hours with nearly 100% sensitvity. However, as a laboratory technique PCR is quite vulnerable to contamination by DNA of other microorganisms. Hence, it is necessary that absolute sterility be observed during sampling, transport and processing of clinical specimens. In addition, evaluation of all test results must be made in relation to positive and negative controls. With its tributes of rapidity and sensitivity, plus being relatively inexpensive, PCR will be likely to find its rightful place among the classic methods of tuberculosis diagnosis

Bu çalışmanın amacı plevral sıvıda Polimeraz Zincir Reaksiyonu (PCR) yöntemi ile M. tuberculosis'in varlığını göstermek ve tüberküloz plörezideki tanisal değerini araştırmaktır. Çalışmaya, SO'si tüberküloz (çalışma grubu), 23'ü tü-berküloz dışı (kontrol grubu) olmak üzere toplam 73 hasta alındı. Çalışma grubu ikiye ayrıldı. Plevra sıvısı yayma, kültür veya plevra doku histopatolojisinden herhangi birisi pozitif olanlar kesin tüberküloz plörezi olarak adlandırıldı. Sadece antitüberküloz tedaviye yeterli cevap kriteriyle tanı konan olgular şüpheli tüberküloz plörezi olarak adlandırıldı. Hastaların plevra sıvılarında, M. tuberculosis complex DNA'sına özgül IS6110 bölgesi PCR ile araştırıldı ve ürünler hibridizasyon-elektroforez-gel-ultraviyole yöntemiyle gösterildi. PCR sonuçları, plevra sıvısı yayma, kültür, plevra doku histopatoloji ve klinik ta-nıyla karşılaştırıldı. Sonuçlar tüm çalışma grubu, kesin tüberküloz grubu ve şüpheli tüberküloz grubu için sırasıyla şöyleydi: Sensitivite % 52.8, % 72.4, % 29.1, spesifite % 95, % 95, % 95, pozitif prediktif de-ğer % 96.5, % 95.4, % 87.5, negatif prediktif değer % 43.1, % 70.3, % 52.7, toplam geçerlilik % 64.3, % 81.6, % 59.0. PCR, direkt yayma, kültür, plevra doku histopatoloji-sinden daha sensitivdir. Uygun primerler kullanılarak 24 saat içinde % 100 hassasiyetle basil DNA'sını saptamak mümkündür. Ancak, PCR'ın DNA açısından kontaminasyon riski yüksektir. Bunu önlemek için örnek alınması, taşınması ve laboratuvar aşamasında steriliteye dikkat etmek gerekir. Örnekleri çalışırken muhakkak negatif ve pozitif kontrollerle karşılaştırarak değerlendirme yapılmalıdır. PCR, bütün bu özellikleriyle ve de ekonomik olması nedeniyle tüberküloz plörezinin hızlı tanısında klasik tanı yöntemleri arasına girmeye adaydır
