Effects of immunoglobulin G on surgical sepsis and septic shock

Abstract

Intravenous administration of high dose Immunoglobulins (Ig) may increase immune defence by providing large amounts of antibodies. The purpose ofthis prospective, randomized, placebo-controlled study was to evaluate the effectiveness if IgG on mortality for septic, high risk surgical patients undergoing intensive care treatment. Forty septic patients receiving intensive care treatment were randomly taken into two groups at Gülhane Military Medical Academy General Surgery Department. Both groups were similar in initial sepsis and APACHE II scores, sepsis score 16 or higher. Study group of 21 patients received high dose IV Immunoglobulin, so Human Albumin was administrated for control group of 19 patients. At the end of the study, significantly lower mortality was recorded in IgG treated group (14%) than in control group (47%). None of the patients in IgG treated group did not die of septic shock but septic shock was the cause of death 26% of patients in control group. Almost identical proportion of IgG and control patients died of Multiple organ failure (14% to 15%). The results of this study indicate that high-dose IgG improves survival and decreases death from septic shock in surgical intensive care patients.

Özet

Immünglobulinlerin (Ig) intravenöz (IV) yolla ve yüksek dozda verilmesi büyük miktarlarda antikor sağlamak sureti ile immün savunmayı arttırmaktadır. Bu çalışmada yoğun bakım gerektiren yüksek riskli cerrahi hastalarda, IgG nin mortalité oranlarına olan etkisi prospektif, randomize, plasebo kontrollü olarak araştırılmıştır. GATA Genel Cerrahi Anabilim Dalı yoğun bakım ünit esinde tedavi gören 40 hasta randomize biçimde iki gruba ayrıldı. Her iki gruptaki hastalarda başlangıç sepsis skorları ve APACHE II skorları aynı olmuş, sepsis olarak kabul edilen 16 ve daha yüksek sepsis skoruna sahip hastalar çalışma kapsamına alınmıştır. Çalışma grubunu oluşturan 21 hastaya yüksek doz I.V. IgG ve kontrol grubunu oluşturan 19 hastaya Hu-man Albumin verilmiştir. IgG ile tedavi edilen hastala-rın mortalité oranında (% 14) kontrol grubuna (% 47) göre önemli bir azalma saptanmıştır. IgG ile tedavi edilen hastalarda septik şok nedeni ile ölüm hiç sap-tanmazken, kontrol grubundaki hastaların % 26'sı septik şok nedeni ile kaybedilmiştir. Her iki grupta da Multiple Organ Yetmezliği (MOY) nedeni ile olan ölümler aynı olmuştur (% 14'e karşın % 15). Sonuç olarak, yüksek doz IgG yoğun bakım tedavisi gören cerrahi hastalarda septik şoktan dolayı olan mortalité oranlarını azaltmaktadır.
