Comparison of ketoconazole and fluconazole treatments in the patients with mycotic vaginitis

Abstract

Comparison of the clinical effectiveness of fluconazole and ketoconazole in the patients with the mycotic vaginitis. Five hundred-twentyone patients applied to the Kocaeli University Medical Faculty Department of Obstetrics and Gynecology with the complaint of pruritis vulva and vaginal discharge were included in this study of 87 patients who had the diagnosis of mycotic infections, ketoconazole 400 mg once a day for 5 days were used in 43, and Fluconazole 150 mg, once a day were used in 44 cases. Short term (5-15 days) and long term (30-60 days) clinical effectiveness were evaluated. In 87 of the 521 patients (%16.70) mycotic vaginitis had been diagnosed. In 83.90% of the cases, the pathogen was Candida albicans and in 16.10% other pathogens were detecded. Short term clinical effectiveness of fluconazole was 94.87% and ketocanozole was 90% (p>0.05). Long term effectiveness were 86.11% and 82.35% (p>0.05) respectively, Microbiologically, short term effectiveness of fluconazole was 87.17% and effectiveness of ketoconazole was 81.35%. Long term effectiveness were 80.55 and 76.47% (p>0.05) respectively. Long term and short term comparison of the fluconazole and ketoconazole treatments for mycotic vaginitis were statistically unsignificant. But fluconazole has superiority of less side effects and advantage of single dose application.

Mikotik vaginit tanısı konulan hastalarda flukonazol ve ketokonazolun klinik ve mikolojik etkinliğinin karşılaştırılması. Kocaeli Üniversitesi Tıp Fakültesi Kadın Hastalıkları ve Doğum Anabilim Dalı'na vaginal kaşıntı ve akıntı şikayeti ile başvuran 521 hasta çalışmaya alınmıştır. Mikotik vaginit tanısı olan toplam 87 hastadan 43 tanesine ketokonazol oral yoldan tek seferde 400 mg olmak üzere 5 gün, 44 hastaya da 150 mg Flukonazol oral olarak tek doz uygulanmış ve tedavi sonrası 5-15. günlerde kısa dönem, 30-60. günlerde uzun dönem klinik ve mikolojik etkinlikleri istatistiki olarak değerlendirilmiştir. İstatistiki değerlendirmede fischer exact test kullanılmış, p0.05), uzun dönemde ise sırası ile %86.11 ve %82.35 (p>0.05) bulunmuştur. Mikolojik olarak ise kısa dönemde flukonazol'un etkinliği %87.17, ketokanozol'un %82.35 (p>0.05), uzun dönemde sırası ile %80.55 ve %76.47 (p>0.05) olarak saptanmıştır. Mikotik vaginitli olgularda flukonazol ve ketokanozol ile yapılan tedavi protokollerinin uzun ve kısa dönemde karşılaştırılması sonucunda istatistiki olarak anlamlı bir fark bulunamamıştır. Ancak flukonazolun tek doz olarak uygulanması ve yan etkisinin az olması bu ilacın üstünlüğüdür.
