Assessment of clinicopathologic features of the cases with endometrial cancer

Abstract

Introduction: Endometrial cancer is mostly diagnosed in postmenapausalwomen. More than 90 % of the cases were thewomen older than 50 years and the disease reaches its highestincidence in women older than 60 years. The most commonlyaccepted hypothesis about the etiology of endometrial canceris the long-term exposure to high levels of estrogen withoutenough opposed progesteron level, which can be called asunopposed estrogen hypothesis. In our study, in which datawere assessed according to 2009 FIGO staging; the featureslike age, gravida, parity, tumor size, stage and pathologicresult of the cases were reviewed and presented.Material and Method: 123 cases who were operated for endometrialcancer between June and October 2003 at our hospitalwere incorporated in our study. Cases were assessed retrospectivelyin terms of age, gravida, parity, preoperative CA 125level, tumor size, depth of myometrial invasion, lymphovascularinvasion, metastatic lymphoid tissue, atypic cell in peritonealcytology and histopathological results.Results: Mean age was determined as 59.7±8.1 years.According to the results of the histopathological analysis ofprobe and surgical specimens , endometrioid type of endometrialcancer was detected in 111 (90.2 %) and 119 cases (96.7 %)respectively. Lymphovascular involvement was seen in 23cases (18.7 %). In 123 cases to whom bilateral pelvic lymphnode sampling was applied, metastases were found in 8 cases(6.5 %). Mean CA 125 level of the patients who had lymphovascularinvolvement was 36.6 IU/ml (maximum 192 IU/ml),whereas mean CA 125 level of the rest of the patients was 16IU/ml (maximum 98 IU/ml) (p= 60years of age. When cases were analysed with regard to histo- logic grade, 70 cases were evaluated as grade 1 (56.9 %). 60 %of these patients were younger than 60 years. 64.3 % of thepatients whose histologic grade was determined as grade 2and 75 % of those with grade 3 were >= 60 years of age(p

Özet

Giriş: Endometrium kanseri çoğunlukla menopoz sonrası kadınlarda teşhis edilmektedir. Olguların % 90'ından fazlası, 50 yaş üzeri kadınlarda görülmekte ve hastalık 65 yaş üzeri kadınlarda en yüksek insidansa ulaşmaktadır. Karşı konulmayan östrojenler hipotezi olarak da adlandırılan yüksek seviyelerde uzun süreli östrojen maruziyetinin progesteron varlığıyla dengelenememesi durumu endometrium kanserinin etiolojisinde en yaygın kabul edilen hipotezdir. Verilerin 2009 FIGO evrelemesine uygun olarak değerlendirilmesine dayanan çalışmamızda olgularımızın yaş, gravida, parite, tümör boyutu, evresi ve patoloji sonucu gibi özellikleri tekrar gözden geçirildi ve sunuldu.Gereçe ve Yöntem: Hastanemizde Haziran 2003-Ekim 2003 tarihleri arasında endometrium kanseri nedeniyle opere edilen 123 olgu çalışmaya dâhil edildi. Olgular yaş, gravida, parite, preoperatif ca 125 değeri, tümör boyutu, myometrial invazyon derinliği, lenfovasküler invazyon, lenf bezi tümör metastazı, periton sitolojisinde atipik hücre varlığı ve patoloji sonuçları açısından retrospektif olarak değerlendirildi.Bulgular: Çalışma grubumuzda ortalama yaş 59.7±8.1 olarak belirlendi. Probe ve operasyon materyallerinin histopatolojik inceleme sonuçlarına göre sırasıyla 111 (% 90.2) ve 119 (% 96.7) hastada endometrioid tip adenokarsinom saptandı (% 90.2). Lenfovasküler tutulum 23 olguda saptandı (% 18.7). Bilateral pelvik lenf nodu örneklemesi yapılan 123 hastanın 8'inde metastaz saptandı (% 6.5). Lenfovasküler tutulum olan hastaların ortalama CA 125 değeri 36.6 IU/ml (maksimum 192 IU/ml), diğer hastaların ortalama CA 125 değeri 16 IU/ml (maksimum 98 IU/ml) olarak saptandı (p
