Maternal electrocardiography and changes in serum creatinine kinase-MB levels during ritodrine hydrochloride therapy for preterm labor

Abstract

OBJECTIVE: Preterm labor is of importance in obstetric practice as it is associated with considerable perinatal morbidity and mortality. Various treatment modalities has been used to diminish aterine contractions and some has gained more popularity due to relatively low side-effects: Ritodrine, a â-sympathomimetic agent is one of the widely utilized medication. STUDY DESIGN: In this prospective study 40 patients one of whom delivered a twin, diagnosed of having preterm labor, received ritodrine for tocolysis. Serum creatinin kinase MB (CK-MB) levels ; before infusion , between 6 to 12 hours of infusion, and ECG variations; before infusion, between 6-12 hours of infusion and during oral therapy were investigated in order to assess the maternal cardiac side effects of ritodrine administration. RESULTS: During infusion, sinus tachycardia in all, nonspecific U wave in 30 %, significant ST-T changes in 57.5 % were observed. No correlation was detected between chest pain and myocardial ischemia findings. However, CK-MB values detected between 6 and 12 hours was found to be significantly higher than that of preinfusion values according to paired t test. (p
