Lipid profile in stroke as a risk factor and clinical correlation

Abstract

Lipid profile in stroke has been evaluated in different studies which included apolipoproteins and which did not include and lipids and lipoproteins has been suggested as major risk factors in various stroke types and subgroups. It has been stressed for the importance of to know lipid and lipid profile of different populations because of influence of race, gender, age and environmental factors on lipid profile. This study was planned to compare lipid and lipoprotein profile of stroke patients with lipid profile of controls whose had similar risk factors, and reflection of lipid profile to clinical features. Lipid and lipoprotein values of 176 acute stroke patients who admitted to our department, compared with values of 71 control subjects who had similar risk factors. There was not significant difference between the mean age of 176 stroke patients (62.67+10.85) and the mean age of controls (60.42+7.01) (p=0.114). Apo A-1 and HDL -C levels were not different in stroke patients than in controls (p=0.008) where-as total cholesterol (p=0.000), LDL-C (p=0.001), VLDL-C (p=0.008) and triglycerid (p=0.019) were higher than con-trols. In the evaluation of for Toronto Stroke Scale scores of the patients and Apo A-1 and Apo B values, there was a sig-nificant positive correlation between Apo B and Toronto Stroke Scale scores (p=0.033). No correlation was found for Apo A-1. Beside the different lipid and lipoprotein profile in stroke patients and controls, correlation of increased Apo B and and Toronto Stroke Scale can suggest reflection of increased Apo B on clinical profile with a strong relation.

Özet

Strokta lipid profili, apolipoproteinleri içeren ve içermeyen çalışmalarda risk faktörü olrarak çeşitli strok tiplerinde araştırılmış ve strokta major risk faktörleri olarak bildirilmiştir. Lipid profilinin cins, ırk, yaş ve çevresel faktörlerden etkilen-mesi gözönüne alınarak, her toplumun strok risk faktörlerinin belirlenmesi açısından lipid ve lipoprotein profilinin bilin-mesinin önemi vurgulanmıştır. Bu çalışma strok hastalarına ait lipid ve lipoprotein profilinin, benzer risk faktörlerini taşıyan kontrol grubu ile karşılaştırılması ve lipid profilinin klinik özelliklere yansımasının değerlendirilmesi amacıyla planlandı. Akut strok döneminde kliniğimize kabul edilen 176 strok hastasına (131 iskemik, 45 hemorajik) ait lipid ve lipoprotein değerleri, benzer risk faktörlerine sahip 71 kişilik kontrol grubuna ait değerlerle karşılaştırıldı. Hasta grubunun yaş ortalaması (62,67±10,85) ile kontrol grubunun yaş orta-lamaları (60,46+7,01) arasında fark yoktu. Hastaların Apo A-1 ve HDL -C düzeyleri kontrollerden farksız bulunurken, Apo-B (p=0,000), total kolesterol (p=0,000), LDL-C (P=0,001), VLDL-C (P=0,008) ve trigliserid (p=0,019) düzey-leri kontrollerden yüksek bulundu.Hastaların Toronto Strok Skalası ile Apo A-1 ve Apo B düzeyleri arasında yapılan değerlendirmede Apo B ile anlamlı korelasyon bulunurken (p=0,033), Apo A-1 ile korelasyon bulunmadı. Strok hastaları ile kontroller arasında oldukça farklı bir profilde izlenen lipid ve lipoproteinlerden Apo B nin Toronto skoru ile de korele bulunması Apo- B yüksekliğinin klinik profile de yakın ilişki ile yansıdığını düşündürmektedir.
