A case of fever of unknown origin due to temporal arteritis that presents with positional vertigo

Abstract
Fever of unknown origin (FUO) means fever that does not resolve itself in the periodexpected for self-limited infection and whose cause cannot be ascertained despiteconsiderable diagnostic efforts. The differential diagnosis is often different in olderpatients, and presentation of the disease is frequently nonspecific and symptoms aredifficult to interpret. Multisystem disease has emerged as the most frequent cause ofFUO in the elderly, and temporal arteritis is the most frequent specific diagnosis.Temporal arteritis (TA), also known as giant cell arteritis, is the most common vasculitis in the elderly. It involves large and medium-sized blood vessels, with predispo- sition to the cranial arteries. Unlike visual complications, vestibular manifestationsin TA, such as vertigo, have been poorly reported. We report a case of a 65-year-old woman with onset temporal arteritis presentingwith fever, headache, night sweats, and especially positional vertigo symptoms. Herphysical examination was normal. The work-up included a temporal artery biopsy,which showed marked lymphocytic infiltrate in the arterial wall consistent with TA(giant cell arteritis). We decided to present this case because of its unusual presentation and the importance of early diagnosis and therapy.

Nedeni Bilinmeyen Ateş (NBA), tanı koymak için yapılan her türlü işlemlere rağ- men sebebi anlaşılamayan ve kendi kendine sınırlanan enfeksiyonlar için beklenilendönemde geçmeyen ateştir. Ayrıcı tanıda, yaşlı hastalarda, hastalığın seyri klasikolmayabileceği için yorum yapmak güç olabilir. NBA olgularında, yaşlı hastalardaen sık olarak saptanan neden multisistem tutulumlu hastalıklardır. NBA’da sıklıklakonulan spesifik tanı ise temporal arterit (TA)’dır. Dev hücreli arterit olarakda bilinen TA, klasik olarak orta ve büyük çaplı damarlarıtutan, nedeni bilinmeyen bir vaskülittir. Yaşlılarda daha sık görülür. Görme ile ilgilibelirti ve bulguların aksine vertigo gibi vestibuler şikayetler TA’da çok nadir olarakbildirilmiştir. Bu çalışmada, TA tanısı konulan, 65 yaşında gece terlemesi, baş ağrısı,ateş ve özelliklede pozisyonel baş dönmesi şikayetleri olan bir bayan hastayı olguolarak sunuyoruz. Hastaya yapılan temporal arter biyopsisinde TA’yı düşündürenarter duvarında belirgin lenfositik infiltratlar tespit edildi. Bu olguyu sık görülmeyenbir tablo ile seyretmesi nedeniyle ve erken tanı ve tedaviyi vurgulamak amacıylasunmaya karar verdik.
