The Role of the Assessment of Prognosis of Serum IL-6, TGF$ beta $1, and Leptin Levels in Head-Injured Patients

Abstract
Objective: Isolated cytokines from the host response to as the main determinant of after trauma show the severity of trauma and released from the period may increase or decrease. Increased blood levels of Interleukin-6 are usually used as indicators of systemic inflammatory response or postoperative morbidity. The other elevated cytokines (TGF β-1) have also the same antiinflammatory properties. On the other hand, decreasing the level of post-traumatic early period, in the later stages as a result of stimulation of cytokine expression in adipocytes by increasing the level of the rising another mediator is leptin. In this study, serum IL-6, TGF β-1, and leptin levels were to investigate the relationship between the assessment of prognosis in head injured patients.Materials and Methods: The study consisted of 15 healthy control group and 30 patients who present with mild and moderate head injury based on Glasgow coma scale of 9-15. Results: When compared to controls serum leptin levels were found to be significantly lower in patients. IL-6 levels were observed a significant difference between patient and control groups, and in term of 48th hour values were significantly higher than 24th hour levels. 48th hour TGF β-1 values were higher than 24th hour values in patient and control groups values, but not significant. Conclusion: It was thought that the IL–6, TGF β-1 level in patients with head trauma become more distinguished when Glasgow coma scale decreased, and also a significant decline in leptin levels in posttraumatic period might be important for the evaluation of the prognosis.
Özet
Amaç: Travma sonrası gelişen konakçı cevabında esas belirleyici olarak izole edilen sitokinler travmanın şiddeti ve salındıkları döneme ait olarak artış ya da azalma gösterebilirler.Proinflamatuvar özellikteki İnterlökin-6’nın akut travma veya stres sırasında kan düzeyi yükseldiğinden genellikle sistemik inflamatuvar yanıt veya postoperatif morbidite indikatörü olarak kullanılır. Aynı şekilde artan diğer bir sitokin, antiinflamatuvar özellikteki TGF β-1’dir. Öte yandan posttravmatik erken dönemde düzeyi azalan, ilerleyen dönemlerde sitokinlerin adipositleri uyarması sonucu ekspresyonu artarak düzeyi yükselen diğer bir mediatör ise leptin’dir. Bu çalışmada hafif ve orta dereceli kafa travmalı hastalarda prognozun değerlendirilmesine olan katkıları yönünden serum IL-6, TGF β-1 ve leptin düzeylerinin ilişkisini değerlendirmek amaçlanmıştır.Gereç ve Yöntem: Kontrol grubunu 15 sağlıklı, çalışma grubunu ise Glasgow koma skalası 9-15 olan hafif ve orta derecede kafa travması mevcut 30 hasta oluşturdu. Çalışma grubundaki bireylerden kafa travmasını takip eden ilk 24 ve 48. saatlerdeki serum IL-6, TGF β-1 ve leptin düzeyleri ölçüldü. Bulgular: Leptin düzeyleri kontrol grubuna göre hastalarda, ilk 24 saat’e göre 48. saatte anlamlıderecede düşük bulundu. Bu çalışmada IL-6 düzeyleri açısından hasta ve kontrol grubu arasında anlamlı bir fark gözlenmezken, 48.saat değerleri 24. saat değerlerine göre anlamlı yüksekti. TGF β-1’in 48. saat değerlerinde kontrol ve 24. saate göre artış görülmesine karşın istatistiksel olarak anlamlı değildi. Sonuç: Kafa travmalı hastalarda düşen Glasgow koma skalasıyla birlikte artışı daha da belirginleşen IL-6, TGF β-1 ve posttravmatik erken dönemde anlamlı bir düşüş gösteren Leptin düzeylerinin bu hastalardaki prognozun değerlendirilmesi açısından önemli katkılar sağlayacağını düşünmekteyiz.
