The Results of Percutaneous Surgical Release in the Trigger Finger Treatment

Abstract

Objective: Trigger finger is a common cause of pain and disability of the hand. There are pain, swelling, limitation of finger movements and a feeling of triggering in the fingers of the hand. Conservative methods are used initially in the treatment, but in case of unsuccess are performed. In this study, we aimed to evaluate the results of the percutaneous release in the surgery of trigger finger. Materials and Methods: A total of 30 patients who underwent percutaneous A1 pulley release due to trigger finger between May 2016 and December 2016 were included in the study. Patients who had previously undergone conservative treatment, but had not any relief in the complaints despite conservative treatment for at least 3 months were included in the study. Percutaneous A1 pulley release was applied to 32 fingers of 30 patients. At the last follow-up, range of motion of finger joints, subjective pain assessment and patient satisfaction were measured. Results: Thirty patients (93.75%) had successful results. Two patients have insufficient release, so open surgery was performed. Major complications were not observed. There were excellent patient satisfaction in 31 (96.8%) of the patients. Only one patient had recurrent pain in the A1 pulley area. Conclusion: Percutaneous A1 pulley release applied in the treatment of trigger finger is a low-cost and minimally invasive technique with high success rate and excellent patient satisfaction. However, the anatomical marker points should be given the greatest importance to avoid possible neurovascular damage during this procedure.

Amaç: Tetik parmak eldeki ağrı ve sakatlığın en sık nedenlerinden biridir. Eldeki parmaklarda ağrı, şişlik, parmak hareketlerinde kısıtlılık ve tetiklenme hissi görülür. Tedavisinde başlangıçta konservatif yöntemler uygulanırken, başarısız ve ileri olgularda cerrahi yöntemler uygulanır. Bu çalışmada, tetik parmak cerrahisinde perkütan gevşetme sonuçlarının değerlendirilmesi amaçlandı.Gereç ve Yöntem: Mayıs 2016-Aralık 2016 tarihleri arasında tetik parmak nedeniyle perkütan A1 pulley gevşetmesi yapılan toplam 30 hasta çalışmaya alındı. Çalışmaya tetik parmak nedeniyle daha önce konservatif tedavi yöntemleri uygulanan ve en az 3 ay konservatif tedaviye rağmen yakınmaları geçmeyen hastalar dahil edildi. 30 hastanın 32 parmağına perkütan A1 pulley gevşetmesi uygulandı. Son kontrollerde hastaların parmak eklemlerinin hareket açıklıkları, subjektif ağrı değerlendirilmesi ve hasta memnuniyeti ölçüldü.Bulgular: Hastaların 30‟unda (%93.75) başarılı sonuç alındı. 2 hastada yeterli gevşetme yapılamadı bu nedenle açık cerrahi uygulandı. Hastaların hiçbirinde majör komplikasyon izlenmedi. Hastaların 31‟inde (%96.8) inde hasta memnuniyeti mükemmeldi. Yalnızca bir hastada A1 pulley bölgesinde ağrı şikayetinin devam ettiği gözlendi.Sonuç: Tetik parmak tedavisinde perkütan A1 pulley gevşetmesi yüksek başarı oranı ve mükemmel hasta memnuniyeti ile düşük maliyetli ve minimal invaziv bir tekniktir. Bununla birlikte bu işlem sırasında olası damar-sinir hasarından kaçınmak için anatomik belirteç noktalarına azami önem gösterilmelidir.
