Identification of Candidate Biomarker Genes for Diagnosis of Irritable Bowel Syndrome by Bioinformatic Analysis

Abstract
Objective: Irritable bowel syndrome (IBS) is a gastrointestinal (GI) disorder characterized by altering bowel habits in combination with abdominal pain or discomfort without detectable biochemical and structural abnormalities. Although the reasons for the occurrence of the disease are not known, it contains many factors. Understanding the pathogenesis of IBS is very important. In this study, it was aimed to identify the genes and molecular mechanisms involved in the emergence of this disease by analyzing the IBS transcriptome data. Materials and Methods: The microarray data selected for bioinformatic analysis is gene expression data stored with the code GSE36701 in the National Center for Biotechnology Information (NCBI) Gene Expression Omnibus (GEO) database. By using this data set, gene expression analysis, functional cluster analysis, enrichment analysis and pathway analysis were performed. Results: In our results, we have detected that the expression level of HLA-DQB1, CAPN8, RAP2C, AGPAT4, GOLGA8A, UNC5CL, CCL13 and ARHGAP9 genes increased and expression levels of SLC28A2, CPB1, MST1L, ATXN1 and FABP2 genes decreased in the constipation dominant IBS. In diarrhea dominant IBS subtype, CAPN8, GOLGA8A, CCL13 and FCRL5 are the genes whose expression level increases. Decreasing expression levels are found as the HLA-DQB1, ERAP2, MST1L and ATXN1 genes. Conclusion: As a result of this approach, IBS-specific molecular profile has been revealed, the genes whose expression levels increased or decreased compared to control group were identified and it has been determined that the subtypes of IBS have a specific molecular profile.

Amaç: İrritabl bağırsak sendromu (IBS), saptanabilir biyokimyasal ve yapısal anormallikler olmadankarın ağrısı veya rahatsızlık ile kombinasyon halinde bağırsak alışkanlıklarının değişmesiylekarakterize olan gastrointestinal (GI) bir bozukluktur. Hastalığın ortaya çıkma nedenleri kesin olarakbilinmemekle beraber birçok faktörü içermektedir. IBS’nin patogenezinin anlaşılması oldukçaönemlidir. Yapılan bu çalışmada IBS transkriptom verisi analiz edilerek bu hastalığın ortayaçıkmasında rol oynayan genler ve moleküler mekanizmaların tespit edilmesi amaçlanmıştır.Gereç ve Yöntemler: Biyoinformatik analiz için seçilen mikroarray verisi National Center forBiotechnology Information (NCBI) Gene Expression Omnibus (GEO) veri tabanında bulunanGSE36701 koduyla depolanmış olan Gen Ekspresyon verisidir. Bu veri seti kullanılarak gen ifadeanalizi, fonksiyonel kümeleme analizi, zenginleştirme analizi ve yolak analizleri yapılmıştır.Bulgular: Çalışma sonucunda konstipasyon baskın IBS alttipinde HLA-DQB1, CAPN8, RAP2C,AGPAT4, GOLGA8A, UNC5CL, CCL13 ve ARHGAP9 genlerinin ifade düzeyinin arttığını, SLC28A2,CPB1, MST1L, ATXN1 ve FABP2 genlerinin ise ifade düzeylerinin düştüğünü belirledik. Diyarebaskın IBS alttipinde, ifade düzeyi artış gösteren genler CAPN8, GOLGA8A, CCL13 ve FCRL5. İfadedüzeyi azalanlar ise HLA-DQB1, ERAP2, MST1L ve ATXN1 genleridir.Sonuç: Bu yaklaşım sonucunda IBS’ye özgü moleküler profil ortaya konmuş, kontrol grubuna göreifade düzeyleri artan veya azalan genler tespit edilmiş, buna ilaveten IBS’nin alttiplerininde kendineözgü bir moleküler profile sahip olduğu tespit edilmiştir.
