Our Results on Microsurgical Testicular Sperm Extraction in Nonobstructive Azoospermic Patients and Evaluation of the Affecting Parameters

Abstract

Objective: To evaluate the factors affecting the success in patients who underwent microsurgicaltesticular sperm extraction (micro-TESE) for nonobstructive azoospermia (NOA).Materials and Methods: Age, duration of infertility, operation history, serum follicle stimulatinghormone (FSH), total testosterone levels in 81 patients and the effect of these parameters on thesuccess in sperm retrieval were evaluated retrospectively.Results: Micro-TESE positivity was observed in 37 (45.7%) and TESE negativity was detected in 44(54.3%) patients. Patients with a mean age of 33.8±6.6 (22-60) were divided into 2 groups accordingto their age. By comparing of these two groups in terms of sperm positivity in micro-TESE, nosignificant difference was found. (P=0.778). The duration of infertility were between 1-21 years. Eight patients (9.8%) had genetic anomalies. TESE positivity was found to be significantly lower inKlinefelter patients (P=0.029) The mean FSH was found to be significantly lower in micro-TESEpositive patients (P=0.04) and there was no statistically significant difference in testosterone levels (P=0.109). While testicular size was observed having no effect on sperm retrieval (P=0.3), theprobability of finding sperm was found to be lower in those with maturation arrest, Sertoli cell onlyand fibrosis in histopathological evaluation compared to hypospermatogenesis.(respectively; 35%,26%, 14.9%, 91%) Conclusion: While there is a statistically significant relationship between sperm retrieval successand accompanying genetic disease, serum FSH levels and testicular histopathology in micro-TESE,the relationship betweenother parameters and the effect of spouse age on pregnancies with spermobtained aren't clear.

Amaç: Nonobstruktif azoospermi (NOA) nedeniyle mikrocerrahi testiküler sperm ekstraksiyonu(mikro-TESE) uygulanan hastalarda başarıyı etkileyen faktörlerin değerlendirilmesi.Gereç ve Yöntem: 2016-2020 yılları arasında NOA tanısıyla mikro-TESE uygulanan ve verilerineulaşılan 81 hastanın yaşı, infertilite süreleri, geçirdiği operasyonlar, serum follikül stimülan hormon(FSH), total testosteron düzeyleri, testis boyutları, testiküler histopatolojik bulguları ve buparametrelerin mikro-TESE’de sperm elde etme başarısı üzerine etkileri retrospektif olarakdeğerlendirildi.Bulgular: 81 hastanın 37 (%45.7)’sinde TESE pozitifliği; 44 (%54.3)’ünde TESE negatifliği saptandı.Yaşlarına göre 2 gruba (22-34 ve 35-60) ayrılan hastalar arasında sperm pozitifliği açısından anlamlıfark saptanmadı. (P=0.778). İnfertilite süreleri 1-21 yıl arası saptandı. Sekiz hastada (%9.8) genetikanomali mevcut (6 Klinefelter sendromu ve 2 AZFC delesyonu) olup, TESE pozitifliği Klinefelterhastalarında anlamlı düşük saptandı (P=0.029). Hormon profiline göre; mikro-TESE pozitifhastalarda FSH ortalaması anlamlı düşük saptanırken (P=0.04), total testosteron düzeylerindeistatistiksel olarak anlamlı bir fark bulunmadı (P=0.109). Testis boyutlarının sperm bulma üzerineetkisi saptanmazken (P=0.3), histopatolojide maturasyon arresti, Sertoli cell only ve fibrozis tespitedilenlerde sperm bulma olasılığı hipospermatogeneze göre daha düşük saptandı (sırasıyla; %35,%26, %14.9, %91) Sonuç: Mikro-TESE’de sperm elde etme başarısı ile eşlik eden genetik hastalık, serum FSHdeğerleri ve testiküler histopatoloji arasında istatistiksel olarak anlamlı bir ilişki mevcutken; hastayaşı, infertilite süresi, testis boyutları, serum testosteron değerleri ile ilişkisi ve elde edilen spermlerleoluşacak gebelikler üzerinde eş yaşının etkisi ise net değildir.
