The Effectiveness of PET/CT in the Regional Evaluation of Breast Cancer

Abstract

Objective: In the staging of breast cancer, Fluorine‐18 fluorodeoxyglucose positron‐ emission tomography (PET/CT) is used especially for detecting distant metastasis, as well as for evaluating regional status. Conflicting results are seen in studies evaluating both the axilla and the receptor status of the tumor in PET/CT. In this study, we aimed to perform the efficacy of PET/CT in evaluating both axilla and receptor status of the primary tumor. Materials and Methods: Between January 2015-June 2017, seventy-five patients, diagnosed breast cancer and performed PET/CT before neoadjuvant therapy, were included in the study. All the patient’s data were analyzed retrospectively. Patients with distant metastasis, non-axillary lymph node metastasis and given neoadjuvant therapy before PET/CT were excluded from the study. Results: While 53 of the 75 patients in the study had lymph node metastasis histopathologically, 22 of them did not. According to the PET/CT, 36 patients were positive and 39 were negative. The sensitivity of PET/CT in evaluating the axilla was 62.3%, while specificity, accuracy, positive predictive value and negative predictive value were 86.3%, 69.3%, 91.6% 48.7% respectively. No significant difference was observed between the luminal groups in terms of SUVmax. As the positivity of estrogen and progesterone receptors in the tumor was increased, SUVmax was decreasing. Conclusion: Although the specificity of PET/CT in evaluating the axilla is high, its sensitivity was no longer found to be sufficient. In addition, although the SUVmax value of the primary tumor differed in terms of histological receptors, it is not sufficient to predict.

Özet

Amaç: Meme kanserinin evrelemesinde Fluorine‐18 florodeoksiglukoz pozitron emisyon tomografi (PET/BT) özellikle uzak metastazların saptanması için kullanılmasının yanında bölgesel durumu değerlendirmede de kullanılmaktadır. PET/BT’nin hem aksillayı hem de tümörün reseptörsel durumunu değerlendiren çalışmalarda çelişkili sonuçlar görülmektedir. Bu çalışmada, PET/BT’nin hem aksillayı hem de primer tümörün reseptörsel durumu değerledirmedeki etkinliğinin incelenmesi amaçlandı. Gereç ve Yöntem: Ocak 2015-Haziran 2017 tarihleri arasında meme kanseri tanısı almış ve neoadjuvan tedavi almadan PET/BT çalışılan 75 hasta çalışmaya dahil edildi. Hastaların tüm verileri retrospektif olarak incelendi. Uzak metastazı olan, aksilla dışı lenf nodu metastazı olan ve PET/BT öncesi neoadjuvan tedavi alan hastalar çalışma dışı bırakıldı. Bulgular: Çalışmadaki 75 hastanın 53 ünde histopatolojik olarak lenf nodu metastazı varken 22 tanesinde yoktu. PET/BT sonuçlarına göre 36 hasta pozitif ve 39’ u negatif idi. PET/BT’nin aksillayı değerlendirmedeki sensitivitesi %62.3 spesifitesi %86.3 doğruluk %69.3 pozitif prediktif değer %91.6 ve negatif prediktif değer %48.7 idi. Luminal gruplar arasında SUVmax açısından anlamlı farklılık izlenmedi. Tümördeki östrojen ve progesteron reseptörlerinin pozitifliği arttıkça SUVmax azalıyordu. Sonuç: PET/BT’nin aksillayı değerlendirmedeki spesifitesinin yüksek olmasına rağmen sensitivitesi yeterli seviyede bulunamamıştır. Ayrıca primer tümörün SUVmax değeri histolojik reseptörler açısından farklılık sergilese de tahmin etmek için yeterli değildir.
