Association Between Antibiotic Resistance and Biofilm Formation of Escherichia coli Strains Isolated from Blood Culture

Abstract

DOI: 10.26650/experimed.2019.19013Objective: Bloodstream infections (BSIs) appear to be important causes of morbidity and mortality. Escherichia coli (E. coli) causes various serious diseases such as BSIs. The emergence of multidrug resistance in E. coli has become a global concern. E. coli has the capability to colonize and survive on several surfaces in different time periods. This is achieved by adhering to inert and cellular substrates and forming a biyofilm layer. The aim of this study is to investigate the relationship of biyofilm formation to antibiotic resistance in E. coli isolates which are isolated from hemocultures.Material and Method: E. coli strains were isolated from blood samples of patients with bacteremia who were hospitalized in intensive care units and in other departments of Istanbul University-Cerrahpaşa, Cerrahpaşa School of Medicine Hospital. Blood cultures were analyzed with the Bactec 9120 system (Becton Dickinson, USA). The identification and antimicrobial resistance of 62 E. coli strains were determined by the Phoenix automated system (BD Diagnostic Systems, Sparks, MD) according to the European Committee on Antimicrobial Susceptibility Testing (EUCAST) and the Clinical and Laboratory Standard Institute (CLSI). Biofilm formation was determined by the Congo red agar method.Results: Biofilm formation was detected in 42 (67.7%) of the sixty-two E. coli isolates. Extended-spectrum beta-lactamase (ESBL) production was 38 (61.2%) and carbapenemase production was 12 (19.3%). 28 (66.6%) of the biofilm-positive isolates were ESBL-positives and 10 (23.8%) were carbapenemase-positives. Nine isolates were both ESBL and carbapenemase positives. Ten isolates were biofilm, ESBL and carbapenemase were positive at all. Conclusion: Resistance rates against various antibiotics in E. coli strains isolated from blood cultures were found higher in biyofilm positive strains than in negative strains at Istanbul University-Cerrahpaşa, Cerrahpaşa School of Medicine Hospital.Cite this article as: Halipçi Topsakal HN, Aydoğan O, Özdemir S, Köksal Çakırlar F. DOI: 10.26650/experimed.2019.19013Amaç: Kan dolaşımı infeksiyonları (KDİ) morbidite ve mortalitenin önemli nedenlerindendir. Escherichia coli (E. coli), KDİ gibi çeşitli ve ciddi hastalıklara neden olmaktadır. E. coli'de çoklu ilaç direncinin ortaya çıkması küresel bir endişe haline gelmiştir. E. coli, farklı yüzeylere kolonize olma ve biyofilm oluşturma yeteneğine sahiptir. Bu çalışmanın amacı kan kültürlerinden izole edilen E. coli izolatlarında biyofilm oluşumunun antibiyotik direnci ile ilişkisini araştırmaktır.Gereç ve Yöntem: İstanbul Üniversitesi-Cerrahpaşa, Cerrahpaşa Tıp Fakültesi Hastanesinin yoğun bakım ve diğer servislerinde yatan bakteriyemili hastalardan laboratuvarımıza gönderilen kan kültürü örnekleri BACTEC 9120 kan kültür sistemi ile analiz edilmiştir. İzole edilen 62 E. coli’nin tanımlanması ve antimikrobiyal direnci, PHOENIX - Otomatik Bakteri İdentifikasyon ve Antibiyotik Duyarlılık Test Sistem ile belirlendi ve şüpheli dirençler E-Test ile doğrulanmıştır. Antibiyotik duyarlılıkları European Committee on Antimicrobial Susceptibility Testing (EUCAST)’a ve Clinical and Laboratory Standard Institute (CLSI)’ye göre değerlendirilmiştir. Biyofilm oluşumu Congo red agar yöntemi ile tespit edilmiştir.Bulgular: Altmış iki E.coli izolatının 42’sinde (%67,7) biyofilm oluşumu tespit edilmiştir. Genişlemiş spektrumlu beta-laktamaz (GSBL) üretiminin 38 (%61,2) ve karbapenemaz üretiminin 12 (%19,3) olduğu bulunmuştur. Biyofilm pozitif izolatların 28 (%66,6)’sı GSBL pozitif ve 10 (%23,8)’i karbapenemaz pozitif bulunmuştur. Dokuz izolat hem GSBL hem de karbapenemaz pozitif bulunmuştur. On izolat ise biyofilm, GSBL ve karbapenemaz üçü birlikte pozitif bulunmuştur. Sonuç: Cerrahpaşa Tıp Fakültesi Hastanesinde kan kültürlerinden izole edilen E. coli izolatlarında çeşitli antibiyotiklere karşı direnç oranları biyofilm pozitif suşlarda, negatif suşlara göre daha yüksek bulunmuştur. Bu sonuçlarla biyofilm oluşturan izolatların antibiyotiklere karşı daha yüksek oranda direnç gösterdiği tespit edilmiştir.Cite this article as: Halipçi Topsakal HN, Aydoğan O, Özdemir S, Köksal Çakırlar F.
